我國居家照護政策執行之評估

Abstract

台灣近十年來老年人口比率每年約以0.4％持續增加，根據經建會的民國97年至145年人口推計簡報指出：我國由高齡化社會過渡到高齡社會只需短短的24年時間，我國老年人口老化的程度，相較於其他先進國家皆快速許多，相對的政府與社會因應及準備高齡社會來臨的時間也縮短了許多，因此將我國居家照護政策執行狀況做深入的研究和分析，是有其必要性，此為本篇論文的研究背景跟動機。 而政府提出因應人口老化問題的相關政策中，以2007年行政院提出的「長期照護十年計畫」最為重要，其中的居家照護服務，是目前推行長期照護政策中使用比率最高的服務類別，因此本論文以我國十年長照中居家照護之執行現況為研究主軸，分析日本、瑞典、德國、美國等國居家照護之執行情形，並分析國內居家照護相關文獻，再利用深度訪談及問卷調查對政策利害關係人進行政策評估，進一步再透過比較分析法來說明我國跟其他國家之差異，利用這三種研究方法做為本論文的研究方式。 本研究發現我國居家照護政策目前在執行上，雖已有一定的成效，有效減輕政策關係人照護時間和金錢上的負擔。但也有人力資源不足、宣傳成效不佳、專業人力不足、政策窗口不一等執行現況，這些都是目前我國居家照護政策執行上所面臨的困境。最後本論文以研究發現及比較各國推行居家照護政策經驗提出建議，分別從政策執行及宣導、政策資源分配兩方面切入，像是在執行與宣導部分1.建立統一窗口；2.適當的宣傳管道；在政策資源分配部分1.定期做人力供需調整；2.鼓勵相關單位結盟與產業投入；3.專業人力制度的建立；4.外籍看護工制度的擬定。

Taiwan is becoming an ageing society rapidly. The government announced a ten-year long term care program in 2007 to deal with the resulting social welfare issue. This study focuses on the best utilized category of the program, home care program. Cross national experiences, including Japan, Sweden, Germany and US, are analyzed. Stakeholder perception of the implementation of home care program is surveyed. The study finds the following weaknesses of the policy implementation: 1.Lack of human resources. 2.Lack of publicity. 3.Short supply of professional personnel. 4.Multiplicity of policy providers. Improvement of the policy implementation is suggested: 1.Establishment of a unified window of service providers. 2.Adequate public relations mechanism. 3.Regular adjustment of human resources. 4.Encouraging investment of related industry. 5.Establishment of professional manpower. 6.Establishment of foreign home care worker system.
