Abstract: | 皮膚是人體當中最大的器官,包裹在身體的表層,扮演著保護人體的重要角色。對於大面積嚴重的皮膚傷害,除了必須及早清創移除壞死組織,也必須給予傷口適當的覆蓋保護。當發生燒燙傷或爆炸意外,病況稍嚴重的患者會需要大體皮膚作為生物敷料,用以減輕患者的痛楚,並保護傷口、加速癒合。使用捐贈者的皮膚(大體皮膚),仍舊是最理想的生物性敷料。因此,設置皮膚組織保存庫,處理及保存大體皮膚,在治療大面積皮膚傷害,就顯得非常重要了。近幾年來,台灣發生數起重大爆炸意外事件,造成大量大面積受傷病患,於是提升國內皮膚保存庫品質的計畫,有其必要性。
本篇論文深入研究皮膚保存方法,發展標準化、一致化的高品質保存流程,並實作出一套雙語介面的大體皮膚保存管理系統,除了提供醫師與皮膚庫人員在皮膚入出庫的記錄與管理上更為便利之外,還具備紙本資料電子化的優勢。目前先以臺大醫院為目標實施,整合醫院資訊系統,以及捐贈者與受贈者電子病歷後,有良好的成效。因此,期望藉由本篇論文的研究成果,在健全皮膚保存庫的架構下,提供量足質精的大體皮膚,造福更多大面積皮膚傷害的病患。 Skin is the largest organ of the integumentary system in the human body, wrapped in the surface of our body, and plays an important role in protecting the human body. For large areas of serious skin damaged, necrotic tissue must be removed by early debridement and the wounds must be given adequate coverage protection. In the event of a burn or blast accident, a severely injured patient may require cadaveric skin as a biological dressing to relieve the pain, protect the wound and speed healing. Using the donor’s skin, which is also called cadaveric skin, still remains the most ideal biological dressing. Moreover, it is important to set up a skin bank to handle and preserve the cadaveric skin for treating large areas of skin damaged patients. In recent years, there have been a number of major explosions in Taiwan and a large number of wounded patients. Hence, upgrading the quality of the national skin bank is necessary. This study was intended to conduct the research on skin preservation methods. We devoted to developing standardized and consistent high-quality preservation procedures and proposing a set of preserving management system with a bilingual interface for cadaveric skin. In addition, it not only provides more convenient way in the recording and management of skin inbound and outbound for physicians and skin bank staff, but also has the advantage of using electronic record with this system. The system was implemented initially at National Taiwan University Hospital. To integrate hospital information system and establish a technological and informational skin bank. After integrating both donors’ and recipients’ electronic medical records, it shows productive results. Therefore, it is our goal to provide a large amount of skin with high quality under the framework of the well organized skin bank, and benefit more patients suffering from extensive skin lesions. |
