dc.description.abstract | 腦血管疾病患者的平衡問題是導致其長期失能的主要原因之一,而現今物理治療觀念,強調任務取向的動作再學習訓練。平衡相關動作屬於任務的一種,因此平衡訓練可視為一種動作再學習,是動作學習理論的臨床應用。2004年Guadagnoli等人提出了挑戰點架構(Challenge point framework),認為動作學習過程中資訊量的多寡決定學習成效之好壞。而動作學習過程中的資訊量則主要由功能性任務難度來決定,若是任務過難或過易時,資訊量則太多或太少而使得學習效益不彰。唯有任務難度適切時,方可提供最恰當的資訊量,而達到最有效的學習成效,此任務難度即為該個案之最佳挑戰點。該理論若為真,則對於促進腦中風患者的平衡訓練效益將有極大之助益,然而目前尚缺乏臨床實證研究。因此本研究期待透過亞急性期中風患者對不同難度平衡訓練的反應,尋找對亞急性期中風病人有最佳療效的平衡訓練學習難度,並將此結果與挑戰點架構之預測對照,以探討挑戰點架構理論的正確性。
本研究設計為個案數為一之隨機試驗(N-of-1 randomized controlled trial),以亞急性期中風患者為研究對象。研究經初步篩選個案能力後,納入三名程度不同之個案,簽署受試者同意書。個案連續接受五天介入前前測,項目包括柏格氏平衡量表、單腳站立與步行速度。之後使用電腦適性化平衡量表(Balance CAT)判別個案平衡能力,據該測驗結果擬訂個別化之低、中、高三種難度的平衡介入。中難度為個案電腦適性化平衡量表得分對應的任務,低難度任務為量表中最簡單項目和個案目前能力項目的中間點任務,高難度任務為量表中最難項目和個案目前能力項目的中間點任務。一種難度介入一週,每週五天每次三至四十分鐘,介入順序以抽籤方式隨機決定。每日的治療結束後立即施測單腳站立、柏格氏平衡量表與步行速度。數據使用斜率分析觀察各期變化趨勢,使用重複量測變異數分析或傅萊德曼檢定比較單腳站立、步行速度數據。 三名個案中起始能力最差者,中難度訓練效果最佳;起始能力次之者,中、高難度訓練效果接近;起始能力最佳者,高難度訓練效果最佳。推論個案的最佳平衡任務難度,可能取決於其本身起始程度的好壞。本結果在個案程度較差時,支持挑戰點架構之假說,但在個案程度提升後則不支持。然而試驗過程中無法控制的常規治療內容、環境改變、個案數為一之隨機試驗可能的治療成效累積效果,都將造成誤差,因此解讀、應用結果時應加倍謹慎。 | zh_TW |
