Abstract: 下肢靜脈曲張(Varicose veins)為現代最常見的一種疾病之一,長期站立及久 坐在下肢靜脈曲張的發生中扮演重要的角色。根據統計,約有 20%的患者兩年內 無法痊癒,更有 66%的患者經歷潰瘍後超過 5 年無法康復。由於症狀輕微時不易 察覺而被許多患者輕忽,直到造成嚴重併發症後才就診,抑或就診後無持續保養與 改善生活習慣,導致嚴重深層靜脈栓塞(Deep vein thrombosis)使得致死風險增加。

過去研究,針對靜脈曲張之保守治療多使用醫療用壓力襪。若踝關節位置的壓 力並非整個壓力襪壓力分佈最大之處,便無法造成適當的壓力梯度,使得靜脈血液 得以回流。然而目前尚未有文獻能精確地以非接觸式量測人體穿上壓力襪後是否 能夠受到梯度壓力。

數位影像相關法為近來新興技術之一,其量測原理為利用數值分析比對變形 前後之影像,以獲得影像上欲量測之關注區域 (Region Of Interest, ROI) 的位移與 應變等資訊。因此,本計畫利用數位影像相關法研究壓力襪施加於小腿的表面應變 分佈,探討壓力襪對於不同腿型之影響以及壓力梯度是否適合穿戴者。

本研究成功地測量到壓力襪下皮膚的表面應變。且觀察到在不同分級壓力襪 以及性別差異下,到個體間小腿表面應變可能存在差異,並且穿著不同級別壓力襪 的小腿應變表現有所不同。這些結果為壓力襪的選擇和使用提供實際參考的價值。
Varicose veins are a common condition in the lower extremities, worsened by prolonged standing and sitting. Medical compression stockings are a conservative treatment, applying graduated pressure from the ankle to the shank to promote venous blood flow. However, non-contact methods to accurately measure the gradient pressure's effectiveness after wearing compression stockings remain lacking in the literature.

Digital Image Correlation (DIC) is a non-contact optical measurement technique used to quantify the full-field surface deformation and strain of an object. Therefore, the study uses the digital image correlation method to measure the surface strain distribution of the lower legs in the compression stockings and explore the impact of leg shapes in the different compression stockings.

This study successfully measured the surface strain of the skin under compression stockings. Variances in individual strain responses were noted across different levels of compression stockings and between genders, leading to distinct shin strain patterns based on the chosen compression level. These findings offer valuable practical insights for the appropriate selection and utilization of compression stockings.
