Abstract: | 健保實施以前,醫院數量大致呈現穩定的狀態,但健保實施以後,十年間有三成(231家)的醫院退出市場。在這231家中,其中又有七成皆為相對中型的私立醫院,此類型的醫院面臨了諸多困境,包括人員聘請不易、住院收入減少、競爭優勢不足、健保給付緊縮、評鑑標準嚴格。因此本研究希望瞭解台灣私立綜合醫院過往的策略類型為何,並透過個案研究何種策略能改善台灣私立綜合醫院的困境,期盼能提供類似個案醫院性質的醫院在面對現今的醫療環境時可行的策略建議與參考。
本研究採用個案分析法,個案研究能處理不同證據,如文件、人員訪談、 觀察,不同證據間可具有高度互補性,更能拼湊出問題的全貌,雖然個案研究法缺乏量化分析方法上科學嚴密性,但其多面向的證據與思考更為貼近現實世界中經理人所面對的問題樣貌。 本研究根據研究主題與個案背景,以桃園敏盛綜合醫院為個案,試圖透過描繪組織的發展以及決策時間點的時空背景,以Miles & Snow提出的四種策略類型: 前瞻型策略、分析型策略、防禦型策略、反應型策略,探究其過往的策略類型為何,以及其策略類型的轉變與行為。 綜合研究結果,從防禦型及反應型逐步轉型為分析型與前瞻型,根據組織本身的優劣勢來鎖定目標客群或服務類型,發展以病人為中心的醫療服務,並用適當的組織結構來支持,可為台灣私立綜合醫院未來的發展方向,也為本研究最衷心的建議。 The new era of National Health Insurance has impacted the management of the medium and private hospitals significantly. For the purpose of survival and maintaining good performance, the medium and private hospitals need to adopt a suitable strategic management. The purposes of this study are to classify private hospital strategies using the Miles & Snow’s typology, and to investigate which strategies can be adapted to managing change in turbulent times by case study. In the hope that gives some advice that which strategies of management these private hospitals can adopt in response to change of market environment. This project was a case study. Although it lacks strict inference of quantitative analyses, it can deal with comprementary datas at the same time, such as document, personal interview and observation. Therefore, case study can provide various aspects of objective, and it is much closer the problems we face in the realistic world. Based on the themes of the study, Min-Sheng Healthcare was choosed as the case. To investigate which strategies it took to managing change in turbulent times by using the Miles & Snow’s typology. This study shows that private hospital could transform it strategic types from defender and reactor to analyzer and prospector gradually; decide target customer and choose the range and types of services, based on the strength and weakness of organization; develop healthcare services from patients’ respective; change appropriate organizational structure to support strategies. |
