摘要: | 慢性肌腱炎( Tendinosis)為肌腱反覆過度的使用,或者肌腱受傷得不到完整
的康復,使得肌腱產生慢性發炎的現象。造成肌腱炎的常見原因除了同部位肌 腱的過度使用外,外力創傷、骨關節的錯位與脫臼也容易導致肌腱受傷發炎。 當肌腱發炎的時候,患部通常會產生紅腫、疼痛與僵硬的症狀。雖然使用消炎 止痛的藥物可減少患者的痛楚,但由於其機制目前尚不明確,有別於常見之急 性肌腱炎,許多研究希望能夠找出發炎的原因,進而予以適當的治療。 本研究合成氧化透明質酸(Oxidative hyaluronic acid)與己二酸二醯肼(ADH) 交聯水膠作為抗氧化藥物載體控制釋放,並模擬機械應力誘發慢性肌腱炎之概 念,發展單軸循環拉伸應力裝置(cyclic stress device) 對體外培養6 週Sprague- Dawley 大鼠之肌腱細胞反覆施加拉力,誘發肌腱細胞病變建立慢性肌腱炎細胞 模型。建立慢性肌腱炎細胞模型後,利用抗氧化物兒茶素(EGCG)以及吡拉西坦 (Piracetam)做治療研究,氧化透明質酸(Oxidative hyaluronic acid)與己二酸二醯 肼(ADH)交聯水膠結合兒茶素(EGCG) 控制釋放達到治療慢性肌腱炎(Tendinitis) 的目的。本實驗利用氧化開環法合成氧化透明質酸水膠,並利用傅立葉轉換紅 外線光譜儀、能量散射光譜儀與生物相容性測試等來分析材料特性。此外,利 用即時定量聚合酶聯鎖反應分析第一型膠原蛋白、第三型膠原蛋白、骨化及脂 肪化等基因表現。 經實驗結果顯示,氧化玻尿酸(Oxidative hyaluronic acid)與己二酸二醯肼 (ADH)交聯水膠結合兒茶素(EGCG)具有治療慢性肌腱炎(Tendinitis)的效果。 在未來,我們希望將研究成果應用於動物實驗以及人類慢性肌腱炎的治療 之上。 Tendinosis is a degeneration of the tendon’s collagen in response to chronic overuse; when overuse is continued without giving the tendon time to heal and rest, such as with repetitive strain injury, tendinosis results. In addition to repetitive strain injury, trauma、out of joint and osteoarthritis can also cause tendinosis. The symptom of tendinosis are pain, swelling, and stiff. Because of its mechanism which is distinct from the common acute tendinitis is not clear. In spite of the use of anti-inflammatory and analgesic drugs can reduce pain for the patient,many researchers hope to identify the causes of inflammation, and then give the appropriate treatment. In this study, we use a cyclic stress device to applying tension repeatedly to tendon cells of 6-week-old Sprague-Dawley rats to develop a mechanical-induced tendinosis cell model Additionally, we add antioxidant drugs EGCG and piracetam to research their effects. The oxidized hyaluronic acid with aldehyde funtional groups prepared by sodium periodate was crosslinked by adipic acid dihydrazide (ADH) to form hydrogel to be drug carrier for control-release regimen in tendinosis treatment. We use quantitative real time polymerase chain reaction to analyze gene expression type I collagen, type III collagen, ratio of type III collagen and type I collagen, Runx2, Sox9 and PPARr. To investigate the characterization of hyaluronic acid-based hydrogel, the FTIR, EDS, TNBS and biocopatibility test were evaluated. In the future, we hope to apply results of study on animal tendinitis model and treatment of human tendinitis. Additionally, the cyclic stress device that is developed in the study is more widely used in different studies.such as cardiac and muscle related study . |
