異位性皮膚炎(Atopic Dermatitis, AD)在幼兒族群中相當常見,主要照顧責任多由母親承擔,可能對其身心健康造成影響。本研究旨在探討照顧AD兒童對母親健康相關生活品質(Health-related Quality of Life, HRQoL)之影響,涵蓋身體與心理層面。 方法: 本研究資料來自台灣出生世代研究(Taiwan Birth Cohort Study, TBCS),為一具全國代表性之母嬰世代資料庫。研究樣本來自2005年全台89個鄉鎮,採多階段分層系統抽樣獲得。以SF-36量表評估母親之生活品質,並將得分落於第25百分位以下者定義為低生活品質。使用邏輯斯迴歸模型分析AD兒童對母親低生活品質的影響,並調整母親年齡、教育程度、慢性病史及家庭月收入等潛在混雜因子。 結果: 與無AD兒童之母親相比,照顧AD兒童之母親有顯著較高風險呈現低生活品質,無論在身體層面(調整後勝算比[aOR]=1.42,95%信賴區間[CI]:1.25–1.61)或心理層面(aOR=1.25,95% CI:1.10–1.43)皆為顯著。 結論: 本研究利用全國性出生世代資料發現,AD兒童對母親之健康相關生活品質具有負面影響,顯示此族群照顧者之身心健康需求值得重視,應納入公共衛生與照護政策考量。 | zh_TW |

Atopic Dermatitis (AD) is prevalent in young children. The primary caregivers, predominantly the mothers, have the burden of taking care of the AD children. This study aimed to evaluate the physical and mental quality of life (QoL) impact on mothers of children with AD. Method: We surveyed a represented mother-infant cohort in Taiwan, the Taiwan Birth Cohort Study (TBCS) cohort, which involved newborns and their mothers selected through a multistage stratified systematic sampling method across 89 towns in Taiwan in 2005. The SF-36 questionnaire was employed to assess mothers’ QoL, focusing on both physical and mental aspects. For this study, we defined low QoL as a score of the SF-36 that fell into the lowest quartile. Logistic regression was used to determine the adjusted odds ratio (aOR) of having low QoL, considering potential confounders. Result: Mothers with AD children had a higher risk of having lower QoL than those with non-AD children on both physical (aOR: 1.42, 95 % confidence interval (CI): 1.25-1.61) and mental (aOR: 1.25, CI: 1.10-1.43) aspects, adjusting for mothers’ age, education level, chronic disease, and family monthly income. Conclusion: In a nationally representative child cohort study, we found poorer QoL among mothers of AD children in both physical and mental components. | en |
