| dc.description.abstract | "背景:蒙古面臨著越來越大的癌症疾病負擔。癌症是該國第二大死亡原因,佔所 有死亡人數的 24.6%。從發生率來看,乳癌和子宮頸癌是女性最常見的癌症。癌 症照護的管理,需要健康照護領域專業者在跨領域團隊中全面且綜合的整合,並 導入病人的參與。美國國家醫學院呼籲提供高品質的癌症照護,強調以病人為中 心照護的重要性,並透過將病人為中心的照護,達到病人參與。因此,近來鼓勵 健康照護健提供者與病人建立合作夥伴關係,邀請患者共同設計照護計劃並支持 他們進行對疾病的自我管理,並記錄測量結果讓其在不同級別的健康照護組織之 間順利地轉換。因此,我們的研究目的有兩個:一、了解在蒙古健康照護專業人 員與病人,對於以「病人為中心的癌症照護」與「病人參與」這兩個議題,各自 的觀點;二、利用乳癌與子宮頸癌患者對於實際治療環境中參與認知探究對於健 康識能、參與偏好和病患積極度伴隨彼此之間的關係。 研究方法:本研究是在蒙古國家癌症中心進行的,這是唯一一家為全體國民提供 三級癌症照護的專科醫院。針對研究目的一,本研究對 30 名健康照護專業人員及 15 名乳腺癌和子宮頸癌患者進行了質性半結構式的訪談。參與的受訪者,針對他 們對於病人為中心(PCC) 和病人參與的觀點、障礙與促進因素等發議題受訪。針 對研究的目的二,本研究採用橫斷面調查方法來調查乳腺癌和子宮頸癌患者的健 康識能、偏好參與和患者積極度之間的關係。對於質性分析,使用 NVivo 軟體進 行了 6 個步驟的主題分析方法。使用社會科學統計套裝軟體(SPSS) V.19 軟體對量 化數據進行統計分析。 結果:蒙古癌症醫療專業人員和乳腺癌和子宮頸癌患者對以病人為中心的照護和 病人參與有不同的看法。從質性的分析找出了以病人為中心的九個照護面向,進一步歸納為三個分類:健康照護的提供(healthcare delivery)、健康照護專業人員 (healthcare professional) 和醫療人員與病人(healthcare professional with patient)。從 本研究的受訪者得知,達到病人為中心共同障礙是:醫療系統問題,照護的可近 性,病人的經濟負擔領導,缺乏資源,缺乏個案管理師, 醫院的環境,諮詢時間 不足,醫療專業者病人為中心的訓練,病人的不積極參與 and 病人家屬的不適當 參與。另一方面來說可以促成病人為中心的照護包含:團隊合作、支持小組、溝 通、關係建立、與醫院某人有私交以及病患持有健康行動卡。 本研究也分析了影響病人參與的障礙和促進因素。臨床路徑流、溝通、諮詢時間 不夠、文化和信仰、患者抵抗、低通識教育、低健康識能、母語訊息不足和過多的家庭參與等被認為是病人參與的障礙。至於病人參與的促進因素則包含經驗分 享小組、醫師促成、手冊、病人積極和家人陪同就醫等。 本研究也針對 225 名乳腺癌和子宮頸癌參與研究受試的患者進行了量化的分析。 他們的平均年齡為 49.3 ± 9.88 歲,乳腺癌患者佔所有受試者的 57.7%。病人認知 參與項目被為兩個面向測量:病人訊息尋求(PIS) 和病人決策 (PDM)。發現病人健 康識能僅顯著影響病人訊息尋求,並且病人偏好參與證明僅對病人決策有顯著影 響。然而,發現病人積極預測因子顯著影響病人資訊尋求和病人決策(PIS [b=0.22, p=0.00] and PDM [b=0.14, p=0.00]) 。 結論與建議:各階層的醫療照護體系都面臨著實現最佳化以病人為中心的挑戰。 綜合的策略需要在健康照護體系與組織中被適當的執行,使醫療專業者、病人層 級能有效的達到病人為中心的照護。從實際病人實際尋病人資訊尋找與病人決策 參與照護來測量,健康識能的程度,參與偏好,病人積極度等每一個都呈現了顯著的影響。而其中患者的積極度是最重要的預測因素。需要制定全面的策略並迅 速實施,以促進和推動以病人為中心的護理和病人參與的提供。建議衛生政策制 定者提供一個以病人為中心的護理支持的環境。在實行中,1) 在醫療機構層面, 可能包括改善癌症護理的提供路徑; 2) 在醫療人員方面,可能包含採用病人決策 輔助工具,實施針對病人活化介入措施,以及補充使用健康口袋卡; 3) 在病人方 面,可能包含將病人代表納入品管小組裡面。未來的研究應通過探索以病人為中 心的護理的其他方面來加強中心的工作,以及上述行動會改善以病人為中心的護 理的提供和病人的參與。" | zh_TW |
