| dc.description.abstract | "背景及目的 為了因應鎮靜安眠藥使用與非醫療使用日益增加的擔憂,台灣有關鎮靜安眠藥相關的規定隨時間也越來越嚴格。然而,針對台灣一般族群及在各人口學分層中,自陳式的鎮靜安眠藥使用與非醫療使用的趨勢仍舊不清楚。此外,關於鎮靜安眠藥非醫療使用的來源與動機的研究也相當缺乏。利用2014年與2018年台灣全國物質使用調查結果,此研究之目的為:(1) 探討整體以及在性別與年齡分層下,兩波調查過去一年鎮靜安眠藥使用及非醫療使用的趨勢;(2) 並檢查社會人口學特性、其他物質使用和憂鬱程度與觀察到的趨勢之關係;(3) 更進一步探討過去一年自陳非醫療使用的來源與動機為何。 方法 研究資料為2014年與2018年全國物質使用調查,此為一針對12-64歲居民並具全國代表性樣本的調查。在2014年納入共17,837名受訪者(完訪率為62.2%),而在2018年則納入共18,626名受訪者(完訪率為64.6%)。每位受訪者皆須以匿名與輔助式自我訪談方式填答一份涵蓋人口學變項、成癮性物質和鎮靜安眠藥使用等資訊的問卷。鎮靜安眠藥的非醫療使用定義為曾在未獲得醫師處方、使用頻率較高、或使用劑量較大之情況下使用藥物。利用雙尾z分數檢定來檢查兩波調查的盛行率是否有顯著改變,並使用差異中之差異法來確認在趨勢間是否存在性別差異。 結果 從2014年至2018年,台灣過去一年鎮靜安眠藥使用與非醫療使用的整體盛行率皆未有顯著改變。然而,12-34歲年齡組的鎮靜安眠藥使用趨勢以及35-64歲年齡組的非醫療使用趨勢存在著性別差異。在鎮靜安眠藥的使用,男性的盛行率邊緣性顯著地減少0.49%,女性的盛行率則顯著地增加0.90%。且這些趨勢間的性別差異可能與單身與酒精使用的比例改變有關。而在鎮靜安眠藥的非醫療使用,男性的盛行率顯著地減少0.37%,女性的盛行率則顯著地增加0.45%。針對過去一年非醫療使用者,由親戚或朋友來源取得鎮靜安眠藥的情況顯著增加,且最常見的使用動機仍是自我治療。 結論 台灣從2014年至2018年,過去一年鎮靜安眠藥使用與非醫療使用的趨勢在不同的年齡組存在著性別差異。對於非醫療使用者,由親戚或朋友來源取得藥物的狀況增加,且自我治療仍是最常見的動機。我們的研究結果對於介入與預防鎮靜安眠藥的使用與非醫療使用具有重要的意義。" | zh_TW |
