dc.description.abstract"研究背景:人類壽命的延長使慢性病人口增加,在照護上需要花費大量人力及社會資源,對國家經濟造成嚴重負擔。有研究指出慢性病能被有效控制時照護效果會更好,當病人體認到自己對於疾病有管理責任,能夠喚醒病人的積極度,會更願意遵從醫囑,透過團隊型照護與病人的配合,增加病人對自己病情照護的信心,鼓勵病人正視自己在治療過程中的主導權,讓病人更願意表達自己在就醫上的偏好,使醫病之間認知差距縮小,整體醫療品質提升,使照護更以病人為中心。 研究目的:針對慢性病住院病人進行研究,想要瞭解就醫過程中病人的積極性與以病人為中心是否相關,希望可以找到影響病人就醫照護結果與就醫品質的因素。 研究方法:本研究為橫斷性研究設計,以台灣中部大型醫療院所為收案院所,針對年齡大於20歲之慢性病人為收案對象,以自填式問卷收集資料,採面訪或電訪進行訪問,共收集761份問卷,刪除過多未填答之資料,最終使用660份樣本進行分析。以病人積極度為自變項,以病人為中心為依變項,將以病人為中心分為總體「以病人為中心」、「尊重病人的偏好」、「資訊、溝通與教育」、「生理支持」、「情緒支持」、「照護協調」、「家人與朋友的參與」及「連續性與照護轉銜」共八個變項,以病人基本人口特性及就醫情形與健康狀況為控制變項進行討論。描述性統計說明資料分布,推論性統計分為雙變項及多變項分析,雙變項分析使用獨立樣本T檢定(t-test)、單因子變異數分析(One-way ANOVA)觀察樣本情形;多變量分析以複迴歸進行,瞭解在控制病人的基本人口學特性及就醫情形與健康狀況後,病人積極度與以病人為中心的相關。 研究結果:研究收案以男性佔多數(63.3%),多數受訪者教育程度在小學以下(55.7%),以退休或無工作者(71.3%),家戶所得在10萬元以下居多(69.6%)。研究發現病人積極度與病人感受到的「以病人為中心」相關,病人的性別、年齡、教育程度、職業、被醫師治療時長、住院期間陪病者身分與住院期間是否另請看護在病人就診的在不同層面影響病人感受到以病人為中心,雖然病人積極度越高不會增加總體「以病人為中心」的感受(-0.192, p=0.002),但會增加「尊重病人的偏好」(0.283, p<.001)、「資訊、教育與溝通」(0.361, p<.001)、「情緒支持」(0.270, p<.001)、「照護協調」(0.353, p<.001)、「家人與朋友的參與」(0.298, p<.001)、「連續性與照護轉銜」(0.328, p<.001)的感受,且有統計顯著意義。 結論:病人積極度與病人所感受到的以病人為中心相關,病人的不同特質對於病人積極度與以病人為中心不同層面會帶來不同的影響。因此政策制定者應該要將病人積極度對照護品質的影響納入關注,醫療提供者則應該注意會影響病人就醫時感受到不同層面以病人為中心感受的因素,藉此提升整體醫療品質,使病人有更好的就醫經驗。"zh_TW
