| dc.description.abstract | "背景：老人的自殺風險相較其他年齡組來得更高。過去老人自殺行為研究多數為橫斷性研究，雖也有以老人族群自殺行為為主的追蹤研究，但主要注重於自殺的心理與社會危險因子，較少納入自殺生物危險因子的研究。 目的：藉由台北市老人健康檢查的長期追蹤資料，了解生物、 心理及社會因子與老人自殺身亡的關聯性。 方法：本世代研究使用西元 2005 年至 2009 年的台北市居民 65 歲以上老人健康檢查之資料，使用期間內第一次健康檢查紀錄，共有101,586 人，串聯全國死因檔。納入變項包括人口學、社會心理變項、體格檢查與生化檢查等，無缺漏值者共有88998人。以 Cox proportional hazards model分析自殺身亡的相關因素。 結果：男性（adjusted hazard ratio [aHR]=2,79，95%信賴區間1.69-4.59）、收入別政府補助（aHR=2.52，95%信賴區間1.42-4.49）、及輕度心理困擾（aHR=1.69，95%信賴區間1.06-6.59）與自殺風險有關。舒張壓和身體質量指數與自殺風險呈負相關，舒張壓每增加一個標準差時自殺風險下降22% (aHR=0.78，95%信賴區間 0.62-0.97)，身體質量指數每增加一個標準差時，自殺風險下降29% (aHR=0.71，95%信賴區間 0.57-0.89)。BMI體重過輕組別（身體質量指數<18.5 kg/m2）相較體重正常（身體質量指數≧18.5 kg/m2 且 < 24 kg/m2）自殺風險增加 182% (aHR=2.82，95%信賴區間 1.44-5.51)。 結論：台北市老人自殺與男性、心理困擾、與經濟弱勢有關，低舒張壓與體重過輕則是過去未曾發現的老人自殺危險因子。" | zh_TW |
