| dc.description.abstract | 目的:西元1999年9月21日發生的集集大地震撼動全台,位於震央的中部地區受到嚴重衝擊。本研究旨在探討以下幾項議題:(1)災後二十年來的創傷後壓力症狀變化軌跡,(2)客觀地震暴露、主觀地震威脅對於創傷後壓力症狀的影響,(3)覺知社會認可與創傷後壓力症狀、創傷後成長關聯,及(4)覺知社會認可、災難經驗揭露預測創傷後成長的序列中介效果。方法:本研究以台中東勢、南投埔里兩地居民為參與者,分為兩大子研究。子研究一為縱貫研究,參與者為107位長期追蹤居民,平均年齡為60.64±8.08歲,以相關分析、多層次模型分析、潛在類別混合模式分析檢驗地震暴露與創傷後壓力症狀關聯、創傷後壓力症狀長程軌跡,並比較不同軌跡之覺知社會認可程度。子研究二為橫斷研究,參與者為420位居民,平均年齡55.17±11.73歲,以相關分析、Process序列中介模型、路徑分析等方式,檢驗地震暴露、災難經驗揭露、覺知社會認可、創傷後成長的關聯。結果:(1)客觀地震暴露和主觀地震威脅對創傷後壓力症狀有正向影響;客觀地震暴露於災難前期影響較大,主觀地震威脅於災難後期影響較大。(2)整體居民至災後二十年,創傷後壓力症狀下降且症狀比例低;88.8%居民創傷後壓力症狀隨時間降低,11.2%居民症狀依然維持甚有增加趨勢。症狀維持與增加的居民,相較於一直以來症狀低者,覺知社會負向認可程度較高,覺知社會正向認可則無明顯差異。(3)災難經驗揭露、覺知社會正向認可皆和創傷後成長有正向相關,覺知社會負向認可與創傷後成長無相關。(4)先經歷覺知社會正向認可而促進災難經驗揭露的序列中介路徑,相較於相反序列,與樣本呈現的變項關聯更具適配性。討論:針對以上結果,探討客觀地震暴露與主觀地震威脅所造成的不同影響,亦對應過往創傷經驗與社會支持相關研究以梳理覺知社會認可的影響,並將研究結果與創傷後成長理論模型相互對應。最後,探討研究結果的臨床運用與意涵。 | zh_TW |
