牙醫到府居家醫療媒合平台商業計畫

Abstract

隨著台灣超高齡化社會的到來，台灣國家發展委員會推估，2060年全台灣將近有40%為65歲以上長者，因此，台灣政府、企業與民間組織非常積極投入長期照護產業，政府長照2.0政策規畫中，從大型醫院機構、社區醫療照護專業服務至社區關懷據點都廣設相關醫事治療所及醫療接送、日間照護及喘息服務，然而，長照2.0中牙醫到府的口腔居家醫療照護並沒特別受到重視，也因此，衛生福利部又積極推動「國民口腔健康促進計畫」以及「109年全民健康保險牙醫門診總額特殊醫療服務計畫」，希望鼓勵更多牙醫進行居家醫療服務。 衛生福利部統計指出，台灣居家醫療照護有需求者約89萬人，現行全台約有15,127位牙醫師，根據「108年牙醫門診總額執行成果報告」指出，民國108年實際有執行到府牙醫服務的使用者僅1,102人，當年度使用人次為2,641人次，台灣牙醫到府之服務仍有非常大的發展機會，有鑑於此，本計畫將針對台灣牙醫居家醫療服務現況，並參照日本居家牙醫服務、美國牙醫居家牙醫服務與台灣藥師到府服務之營運方式，推出牙醫到府媒合平台計畫，希望藉此促進台灣更多醫護人員可離開醫療院所進行居家醫療服務，促進台灣居家醫療及長期照護產業之發展。

According to the National Development Council, it is estimated that the elders, over 65 years old, will nearly comprise 40% of total population in Taiwan with the coming of super-aged society. Hence, the Taiwan government, enterprises and civil society organization (CSO) put lots of resources into the industry aggressively. Of the Long-Term Care Plan 2.0, the government provides related medical treatment, transportation and pick up service and respite care for hospitals, community of professional medical integrated service center and community of long term care stations. However, there’s one important service missed out. The dental home visit and oral health care, as crucial as the services mentioned above, is not valued enough. Therefore, the Ministry of Health and Welfare is pressing ahead with “National Improving Oral Health Plan” and “Global Budget Scheme on Dental Clinics and Special Dental Care Service Plan”, hoping to encourage more dentists to provide the service of home-based medical care. The statistics conducted by the Ministry of Health and Welfare show that there are approximately 890,000 populations, who are in need of home-based medical care, and the number of total dentists is 15,127. According to “Global Budget Scheme on dental clinics and special dental care service plan” during the year in 2019, only 1,102 dentists provided this kind of service in Taiwan, while 2,641 patients used this service. As a result, “Dental Home Visit and Oral Health Care” has a promising opportunity to grow. In this case, the plan builds the platform helping to bridge the gap between dentists and patients, who need home healthcare, based on the current situation of Taiwan dental home visit, and referred to the cases in Japan and the U.S. Also, take the example of pharmacists' home visit experience in Taiwan. With those efforts, it is hopeful that dentists can be encouraged to leave the hospitals and clinics to serve home-based medical care, facilitating the development of home healthcare and long term care.
