| dc.description.abstract | 由於透析病人需要持續性的透析治療再加上所引發的高額費用，透析服務不論在台灣或是國外都是重要的議題，透析服務包括血液透析與腹膜透析二種模式，二種模式分別在成本以及效果上的比較已有許多研究。在成本效果分析(cost-effectiveness analysis)上大部分以決策分析模型(decision analytic model)進行，缺乏以個人資料(individual data)分析的研究。台灣末期腎臟病發生率與死亡率為世界前幾名，目前健保鼓勵使用腹膜透析，需要血液透析與腹膜透析成本效用分析(cost-utility analysis)作為實證基礎。因此本研究將以個人資料進行成本效用分析。
研究目的 1 比較不同透析模式末期腎臟病人健康相關生活品質。 2 分析不同年齡層腹膜透析與血液透析病患每年平均醫療費用。 3 不同透析模式之成本效用分析。 研究方法 本研究先進行2013年6月至2014年5月止，接受透析病患之個人相關基本資料與2001年至2010年健保資料庫的跨資料庫配對，將以性別、年齡、教育程度、行業別、個人年平均月收入、透析時間與透析病人疾病嚴重程度等變項進行配對。使透析病患的醫療成本能更具代表性，也讓缺乏存活情形的問卷資料更加完整。並以線性迴歸方式比較二種透析模式在生活品質與總成本上差異。最後進行成本效用分析計算二種透析模式間Incremental cost-effectiveness ratio(ICER)與Quality-adjusted life years (QALY)。 結論 整體而言腹膜透析確實比較符合成本效用，使用不同透析模式病患生活品質、醫療成本、存活時間皆有差異，即每多存活一個生活品質調整年(QALY)所需花費的費用腹膜透析是低於血液透析的。但仍需考慮病患自我照顧能力等個人因素，聽取醫師專業建議。 | zh_TW |
