Abstract: | 隨著環保意識的興起以及為了達到節能減碳的目的,自行車的使用率日亦漸增。膝關節相對髖與踝關節在自行車運動過程中,擁有較大的活動度,因此有很高的比例為膝關節的過度使用所苦。在膝關節運動中,由肌肉、韌帶及軟組織等精確的控制,改變關節角度來達到我們所希望達成的動作。其中膝關節的韌帶在運動過程中扮演維持膝關節穩定性及傳遞股骨和脛骨間相對運動學資訊的重要角色。而自行車更廣泛應用於下肢傷害的復健,特別是膝關節十字韌帶損傷患者,在膝關節缺少十字韌帶的情況下,膝關節的穩定性會受到影響。
有鑑於機械工程的理論基礎以及電腦的快速發展,許多研究皆利用有限元素分析的方法來得到非侵入式量測下之三維膝關節韌帶力學資訊。有限元素分析流程式由三大部分所構成,包含幾何模型的建立、材料參數的給定以及邊界條件的變化。骨頭以及韌帶的幾何模型是透過電腦斷層掃描以及核磁共振造影建立,韌帶材料參數由KT-2000膝關節量測儀器取得,邊界條件即是骨頭間相對的運動學變化,是由動態X光掃描後使用影像處理比對的技術得到。因此本研究將以上由本實驗室驗證過的有限元素分析流程,運用在活體實驗上,探討正常年輕人在自行車運動中膝關節韌帶之受力情形以及應力分布。由這套跨領域的方法,將這些詳細的資訊應用在臨床上,在治療、復建以及預防都有非常大的幫助。 本研究結果顯示出,在自行車運動過程中,後十字韌帶為主要承受力量及應力之韌帶,且在踩踏前半圈受力遞減;後半圈則呈現遞增的現象。而阻力的高低對於韌帶受力、最大主應力及應力分布沒有太大影響。由復健的角度來看,不建議後十字韌帶損傷患者使用自行車運動來當作復健流程。 With the growing awareness of environmental protection, the popularity of cycling also grows. The knee ligaments plays an important role not only about joint stability but also transmit the relative kinematics information between the femur and the tibia. Exercises with cycles widely applied to the rehabilitation of the lower extremities especially in patients with cruciate ligament injuries. Finite element analysis procedure consists of the geometry model reconstruction, using computer tomography for bone and magnetic resonance imaging scan for ligaments information, the ligament material properties definition, using the KT-2000 knee ligament arthrometer, the boundary condition variation, using fluoroscopy and computer models image registration. The objective of this study was to analysis the stress distribution and the force of the in vivo knee ligaments in normal young subjects using validated finite element analysis procedure. It will be helpful to use in the treatment, rehabilitation and prevention. The finite element analysis showed that PCL was mainly stretched and stressed during cycling and there were no effect on ligaments force, stress distribution under unloaded and loaded. Bicycling will not appropriate for PCL injury patients. |
