摘要: | 研究背景: 聽力障礙是年齡大於六十五歲以上老年人常見的障礙之一,目前已知最有效且經濟的方式,就是配戴助聽器並配合選佩後的聽覺復健。由於科技的進步,數位化助聽器具備有更新更強大的功能。然而針對老年人使用數位化助聽器的主觀及客觀的結果測量,至今仍未有系統的被評估。
研究方法: 以前瞻性縱貫式研究方式,選用條件符合以下三點的受試者為研究對象:(一)年齡滿六十五歲以上,(二)在台大醫院耳鼻喉部助聽器選配室選配數位化助聽器(三) 完成標準選配流程者。利用客觀的聽力檢查及主觀的問卷調查來測量老年人使用數位助聽器的結果,並分析可能影響滿意度的相關變項。 結果: 共101名受試者完成標準配流程。在結果量測方面,客觀的聽力檢查及主觀的問卷調查均優於選配前。對助聽器的整體滿意度,與國外文獻沒有差別。在滿意度相關變項分析方面,共有聽力障礙的類型、性別、選配前聽力圖的圖形、助聽器的外觀類型與選配前老年人聽力障礙量表得分,五個因子具有顯著意義。 結論:每日使用助聽器的時間與整體滿意度呈現正相關。臨床上,以詢問老年人每日佩戴助聽器的時間來表示對助聽器的整體滿意度,是一個可行且準確的方法。 Hypothesis and Background: Several recent studies have shown that new technologies of modern digital hearing aids increased benefit for people with hearing impairment. In this study, we focus on the impact of new technology on the geriatric population. Objective: To examine the objective and subjective benefit of digital hearing aids in older hearing impaired subjects, and also to find out the possible contributing factors of overall satisfaction Design: Prospective study Method and Subjects: 101 subjects older than 65 years with bilateral hearing loss were included in this study. All subjects were fitted with digital, multi-channel compression hearing aids in our hearing aid clinic since 2005. Hearing aid counseling, fitting, verification, and validation were all performed by the same audiologist according to a standard procedure. Results: In total, 101 (61 male 40 female) out of 162 patients responded to the questionnaire (response rate of 62.35%) from Feb 2005 to Dec 2007. The average age of subjects was 77.95 with a range from 65 to 92. The results of objective audiometric exams and subjective questionnaires revealed benefit for the elderly using modern digital hearing aid. Compared with previous study, overall satisfaction rates were not significant different. For the elderly, five factors, type of hearing loss, gender, audiogram configuration, style of hearing aids and pre-fitting HHIE score influenced overall satisfaction. Conclusion: Daily usage time of hearing aid has a positive correlation with overall satisfaction. Compared with complex questionnaires, asking the elderly “How many hours did you use your hearing aid(s)?” is easier to administer clinically. |
