dc.description.abstract | 諾羅病毒為杯狀病毒科之成員,其基因體為單股正向RNA,由三個開放讀架 (ORF) 組成,在ORF1與ORF2之間有重疊區。ORF1表現包含RNA聚合酶在內等非結構性蛋白質產物,ORF2及ORF3分別轉譯出主要 (VP1) 及次要 (VP2) 結構性外殼蛋白質。諾羅病毒依據聚合酶及外殼蛋白質基因的核酸序列相似性區分為五種基因群 (GI - GV)。與人類疾病有關之GI和GII基因群可分別再細分為8種及17種基因型。諾羅病毒是造成人類病毒性急性腸胃炎的主要致病原之一,所有年齡層皆有可能遭受感染,然而對於嬰幼兒、老年人和免疫功能不全者易造成危害,目前並無有效的抗病毒藥物,感染後也無法獲得終生免疫。根據統計,在開發中國家每年約有二十萬五歲以下孩童死於諾羅病毒的感染。近年來研究顯示諾羅病毒基因組有高度突變和重組的傾向,常導致新興病毒株的產生和大流行。
本研究收集來自台大醫院病毒室1981~1993年及2006~2007年共計3391件糞便檢體,針對外殼蛋白質基因N/S區域之核酸序列 (以下簡稱C區) 進行反轉錄-聚合酶連鎖反應,以檢測是否存在諾羅病毒。PCR陽性檢體再針對部分聚合酶基因序列 (以下簡稱A區) 及C區序列進行核酸定序及基因分型,最後偵測到172個諾羅病毒株,並納入本實驗室之前分析的2002~2005年及2008年初已定序之78個病毒株,進行整體分析,以瞭解北台灣地區諾羅病毒的分子流行病學。 扣除來自相同病人的檢體,239個病毒株中有235個屬於GII基因群 (98.3%),僅有4個屬於GI基因群 (1.7%)。依據A區及C區序列進行基因分型,以GII.4基因型所佔比例最高 (72.4%),共計173株,其次為GII.6,共計22株 (9.2%)。不同年代流行之GII.4病毒株有所差異,1988~1992年屬於Camberwell (或Bristol) 變異株;2002年以 Farmington Hills變異株為主,並於上半年偵測到零星幾個US95/96變異株;2004~2005年間之冬季是Hunter變異株;2006~2008年以2006b變異株為主,2006年尚偵測到 Hunter變異株及零星幾個2006a變異株。另於1980年代早期檢出編號395/84/TW之GII.4病毒株,它不屬於前述任一群,其C區序列經BLAST顯示為CHDC2094/74/US 相似株 (核酸相似度94.3%),但A區序列則接近Bristol/93/UK病毒株 (核酸相似度92.3%)。 A區及C區序列分屬不同型別之疑似重組株共計15個,以GII.b/GII.3和GII.12/GII.4兩種重組型較多。分析10個重組病毒株的ORF1至ORF2核酸序列 (7株病毒已於民國96年莎日娜碩士論文中探討),顯示重組位點在ORF1/2重疊區附近。 本研究顯示北台灣地區在2002至2008年間,諾羅病毒株的基因序列確實有因年度而發生變化,並與國際流行趨勢相似。而本地早期病毒株395/84/TW是屬於70年代 [CHDC2094/74/US] 或80年代 [Camberwell (或Bristol) 群] 之病毒,或是另一種變異株,仍有待後續基因體全長序列分析,以瞭解病毒之演化。 | zh_TW |
