| dc.description.abstract | 研究背景:以往檳榔的相關研究都著重在探討口腔癌,較少探討嚼食檳榔引起的相關代謝反應,近年來有少數的學者發表嚼食檳榔與糖尿病、胰島素阻抗及肥胖有關連,流行病學上也發現在南亞及其他高檳榔嚼食率的地區,糖尿病及代謝症候群的盛行率也較高。台灣的原住民的嚼食檳榔的盛行率高達40%,卻少有研究提及原住民嚼食檳榔及代謝症候的相關性,探討嚼食檳榔、發炎因子及代謝症候群的相關研究更是闕如。
研究目的:本研究的主要目的是探討嚼食檳榔及發炎因子與代謝症候群之相關性,進一步探討嚼食檳榔引發代謝症候群可能的機轉。 研究設計及方法:本研究是個橫斷面的社區篩檢計畫,藉由原住民鄉舉辦的整合式健康篩檢,收錄1433位大於三十歲的原住民作為研究對象,以問卷的方式收集相關變項,代謝症候群以NECP-ATP III的定義作為標準,同時測量研究對象的血液白血球數、腫瘤壞死因子(TNF-α)及leptin的血漿濃度。結果:原住民的代謝症候群盛行率為41.8%(男性為41.1%,女性為42.4%),嚼食檳榔的盛行率為52%(男性45%,女性57%)。嚼食檳榔者其血糖代謝異常、肥胖以及血脂異常的比率皆高於非嚼食者,進一步將檳榔的嚼食量分成三等分,以回歸分析校正代謝症候群已知的危險因子後發現嚼食檳榔與代謝症候群有顯著的相關性且呈現劑量效應。嚼食檳榔者不論男女周邊血液的白血球數皆較非嚼食者高, TNF-α在男性嚼食者顯著的上升,而leptin在女性嚼食者顯著地高於非嚼食者。路徑分析發現嚼食檳榔及發炎因子與代謝症候群的關聯男女有別,女性與leptin有關,而男性則與嚼食檳榔升高TNF-α繼而降低HDL-C有關。 結論:本研究顯示原住民是代謝症候群及嚼食檳榔盛行率皆很高的族群,嚼食檳榔是代謝症候群有意義的獨立危險因子,嚼食檳榔與罹患代謝症候群與否間具有劑量效應;發炎因子在代謝症候群中扮演的角色可能有性別上的差異。 | zh_TW |
