dc.description.abstract | 背景與目的:近年來,不論在國內外皆有學者紛紛大力倡導「以病人為中心」的照護觀念,而相關的研究也陸陸續續有國內、外學者進行中,但綜觀國內外相關文章很少以精神病患為研究主題,去實際了解較為弱勢的精神病患,對「以病人為中心」的照護之信念、感受與期待,期待未來可以依此研究結果,進一步規劃可促成精神病患朝向健康且具理論基礎的介入方式。
方法:本研究的目的為探討以「病人為中心」之重要性認知與達成間的關係,採用橫斷面研究設計。採用結構式問卷,以英國Surrey大學學者Ogden等人2002年於發展的問卷為基礎內容,經過Ogden同意授權翻譯使用後,進行中文翻譯,再由二位精神科專業人員進行兩輪回譯(back translation),再請Ogden就回譯文檢視翻譯品質,就Ogden給予的建議與市醫人體研究倫理委員會的建議進行最後修改,成為中文化問卷。本研究邀請臺北市立聯合醫院松德院區四家社區精神復健中心之學員,以普查法參與本研究。 研究結果: 一、「以病人為中心照護」之重要性:社區精神復健中心的學員與其社區精神復健中心的工作人員間,對「以病人為中心照護」之重要性有差異。病人與工作人員在除了在重要性構面四(資訊給予)沒有顯著差異外,病人與工作人員在其他重要性構面,以及重要性構面總分有顯著差異。 二、「以病人為中心照護」之達成度:社區精神復健中心的學員對認為社區精神復健中心「以病人為中心照護」達成度與社區精神復健中心的工作人員對「以病人為中心照護」之達成度並沒有顯著差異。此外,志工對工作人員評價高於病人的評價。 三、病人「以病人為中心照護」之重要性與達成度之關係:複迴歸分析顯示,在整體構面總分方面,保留教育程度控制變項,以及自變項病人「以病人為中心照護」構面一(工作人員感受性)、構面二(病人參與)重要性分數,進行迴歸分析,達到顯著差異。「以病人為中心照護」之重要性與達成度有關,呈正相關,重要性信念分數愈高,達成度分數愈高。 四、工作人員「以病人為中心照護」之重要性與達成度之關係:工作人員「以病人為中心照護」各個構面之重要性信念,與其相對「以病人為中心照護」各個構面之達成度,皆呈正相關,重要性信念分數愈高,達成度分數愈高。 建議:希望主管單位未來進行醫療機構評鑑時,能將「以病人為中心」列入社區精神復健中心未來評鑑標準,已達成人性化全人照顧之目的。 | zh_TW |

In recent years, the concept of patient-centered care is widely promoted by scholars and many studies were conducted in medical facilities to understand this concept. However, research on this topic for underserved populations in Taiwan remains rare. The current study is to survey psychiatric patients’ perception of “patient-centerness” in hoping to generate an empirical-based intervention that will improve the quality of care for psychiatric patients. Methods This survey study examined the significant concept of “patient-centerness” and its relationship with the degree of accomplishment in mental health facilities. The Chinese version of the “Patient-Centered Questionnaire” (Ogden, Ambrose, Khadra, Manthri, Symons, Vass, et al., 2002) was developed by the researcher under Dr. Ogden’s assistance and was adopted as the research instrument. Data was collected from long-term psychiatric patients who attended the community rehabilitation centers of the Taipei City Hospital, Songde Branch. Multiple regression analysis method was employed. Results Significant differences in degree of comprehension on the idea of patient-centered care were found between patients and staff. However, they did share the same perception of the degree of accomplishment in their facilities. Results also showed that there were positive correlations between the degree of comprehension and the degree of accomplishment both in patients and staff. Practice implications In light of the research results, to employ patient-centerness as a standard evaluation criteria to the community rehabilitation center accreditation procedure is highly recommended. | en |
