| dc.description.abstract | 白內障是導致全球中老年人視力障礙的主要原因之一, 白內障治療方式也逐漸由疾病本身的排除,走向提昇視覺功能及改善病人生活品質的治療型態。近年來人工水晶體發展日新月異,而自2001年以後,非球面人工水晶體的發明,其訴求為減少病患手術後的球面像差,也提供了病患一個新的選擇。
過去在國人植入非球面人工水晶體的研究尚未有文獻發表,我們希望藉由臨床隨機試驗的研究設計,來探討國內植入非球面人工水晶體病患與植入球面人工水晶體病患在視力、高階像差及視覺相關生活品質的差異,我們以台北市某醫學中心的白內障手術病患總計74個眼睛,隨機去接受球面人工水晶體(Acrysof SA60AT),或非球面人工水晶體(AcrySofIQ SN60WF)的植入。我們在收集了病患手術後高階像差及手術前後的視力資料,並以改編過之中文版NEI VFQ-25評估手術前後的視覺相關生活品質。 重要的研究結果如下: 1.置入球面人工水晶體病患手術後最佳矯正視力logMAR值為0.12± 0.08,置入非球面人工水晶體病患手術後最佳矯正視力logMAR為0.09±0.07,兩組沒有顯著差異(0.11)。 2.置入非球面人工水晶體病患手術後球面像差平均為0.33±0.21,置入球面人工水晶體病患手術後球面像差平均為0.12±0.23,兩組有顯著差異(P=0.001)。 3.以問卷評估視覺相關生活品質得分結果,置入球面人工水晶體病患手術前後總得分改變為9.49±5.28;置入非球面人工水晶體病患手術前後總得分改變為11.94±7.80,兩組沒有顯著差異(P=0.16)。但在眩光的次向度得分上,植入球面人工水晶體病患手術前後總得分改變為12.4±18.44;植入非球面人工水晶體病患手術前後總得分改變為22.5±20.60,兩組有顯著差異(P=0.04)。 我們的研究結果顯示出植入非球面人工水晶體可以減少病患手術後的球面像差,並且可以改善病患手術後炫光程度。 | zh_TW |
