dc.description.abstract頭頸部癌症為台灣最常見的十大癌症之一,頭頸部癌症之中又以口腔癌及鼻咽癌為最常見,口腔癌2016年在十大癌症之中排名第五位,鼻咽癌則是在2016年排名在全國男性癌症的第十二位。口腔癌以及鼻咽癌的病患,無論是病程進展、手術、放射線、化學治療等,都會影響到口腔的功能,因此,許多病人都會有吞嚥困難以及營養不良的問題。另一方面,患者在接受治療後,身體心像改變與對於疾病、治療的焦慮,在照護上也相當重要。本研究目的為(1)探討口腔癌、鼻咽癌患者主要面臨之進食困難程度;(2)探討口腔癌、鼻咽癌患者之憂鬱及症狀嚴重之程度;(3)探討進食困難、症狀嚴重度、憂鬱及營養狀況之相關性;(4)探討口腔癌、鼻咽癌營養狀況之預測因素。本研究採用採橫斷式研究設計(Cross-sectional design),於病人完成全部療程後三個月以上至五年內在門診進行收案與評估,並以結構式問卷蒐集資料,包含迷你營養評估量表(MNA)、進食困難量表(CSI)、醫院焦慮憂鬱量表(HADS)內之憂鬱子量表,以及症狀嚴重程度量表(SSS),並以描述性統計以及推論性統計進行研究。研究發現影響進食困難的因子,依嚴重程度依序為:口乾、無法把食物咬爛、吃東西沒有味道。營養部份,則發現本研究約有58%以上的病人有營養風險,其中和治療方式、疾病分期有顯著的關係。本研究以迷你營養評估量表與另三種量表做皮爾森相關分析,相關程度由高至低排列依序為咀嚼問題(r=-0.53, p<0.01)、憂鬱(r=-0.53, p<0.01)、身體症狀(r=-0.49, p<0.01)、吞嚥問題(r=-0.31, p<0.01)、感覺問題(r=-0.26, p<0.01)、舌頭攪拌問題(r=-0.23, p<0.01)。另使用線性回歸分析,營養狀況之預測因子分別為咀嚼問題(ΔR=0.285)、身體症狀(ΔR=0.094)、憂鬱風險(ΔR=0.027)、吞嚥問題(ΔR=0.026)。本研究提供患者現況與實證依據,並期望未來能有夠多研究與介入治療投入於患者營養不良之預防。zh_TW
