dc.description.abstract | 研究背景:懷孕相關下背痛是懷孕時期一個普遍發生的症狀,並且影響婦女的日常生活,特別在第三妊娠期疼痛影響最大。然而現有知識對於疼痛及相關經驗(例如:社會支持、憂鬱、以及疼痛厄運感)的變化、以及影響疼痛變化的相關因素所知仍十分有限。研究目的:因此,本研究目的有二:(1)探討第三妊娠期懷孕婦女下背疼痛強度與疼痛干擾及其相關經驗(社會支持、憂鬱、以及疼痛厄運感)的變化;及(2)探討影響第三妊娠期懷孕婦下背疼痛強度與疼痛干擾變化的因素。方法:本研究使用縱貫性研究設計,於台灣北部一所醫學中心及一所地區醫院的產前門診以連續性取樣方法收案,本研究收案前均獲得IRB同意,本研究使用簡易疼痛量表、病患健康量表、疼痛厄運感量表、以及生理工作負荷量問卷來收集資料,時間點分別為妊娠第28週、第32週與第36週。於第一時間點(妊娠28週)招募238位懷孕婦女,於第三時間點(妊娠36週)時,有200位持續參與。本研究使用描述性統計分析、階層式迴歸分析、以及廣義估計模式分析資料。結果:本研究發現疼痛干擾、憂鬱隨時間而增加,社會支持及疼痛厄運感維持穩定。疼痛經驗變化的預測因素方面,疼痛厄運感、妊娠第24週的疼痛強度、以及時間(由妊娠第28週進行至第36週)均可預測懷孕相關下背痛疼痛強度的變化。此外,社會支持、憂鬱、疼痛厄運感、疼痛強度、以及時間(由妊娠第28週進行至第36週)均可預測疼痛相關下背疼痛干擾的增加。本研究也發現在控制憂鬱對疼痛的影響力之下,疼痛厄運感仍會影響懷孕相關下背痛的疼痛強度與疼痛干擾。結論:本研究應用生理-心理-社會模式於第三妊娠期婦女下背痛的型態變化,發現憂鬱與疼痛厄運感在懷孕相關下背痛的發展上扮演重要的角色,以及社會支持可能的風險,本研究找出這些變數,可以協助健康照護人員減緩懷孕婦女的懷孕相關下背痛的強度與干擾他們生活的強度,進一步增進婦女健康。建議未來研究探討憂鬱、疼痛厄運感與社會支持對於持續形成至產後慢性下背痛的可能影響。 | zh_TW |
