摘要: | 患有纖維肌痛症(Fibromyalgia, FM)的慢性疼痛患者長期受到疼痛所苦。除此之外,他們也面臨認知功能的缺損。這種「認知缺損」可能源自於疼痛佔用了他們的注意資源。正念訓練的介入已被研究證實在緩解疼痛、增強認知功能及調節情緒方面有正向的效果。然而,正念訓練對於執行注意力的關係,在健康正常人以及臨床診斷的病人之間的比較研究上,出現不一致的結果。有學者提出,與健康正常人(對照組)相比,正念訓練在注意力控制上的效果,可能對患有注意力缺陷的人有更明顯的影響;然而,這一觀點需要進一步的研究。再者,研究也發現正念訓練對於執行注意力的效果,會受到情緒調節及疼痛的影響。為了探討這些部分,本研究對纖維肌痛患者(FM)和健康對照組(HC)進行了為期 4 週的正念減壓訓練(mindfulness-based stress reduction, MBSR),以探討兩組參與者在注意力、情緒、疼痛感的變化。本研究利用數字史楚普干擾測驗、情感標注測驗、痛覺問卷來評估正念訓練介入前後差異。結果顯示,健康對照組在正念減壓訓練後,相較於患者所需要進行認知作業、情緒辨識時的反應時間減少。兩組人在反應時間上的差異,可能由兩個潛在原因引起:(1)不同的疼痛和情緒干擾,以及(2)不同的正念特質。首先,纖維肌痛患者的疼痛和情緒症狀繼續對認知作業造成挑戰。其次,纖維肌痛患者較低的正念特質,可能會減弱他們當下意識的專注,及接納包容在正念練習時出現的情緒。因此,纖維肌痛患者可能需要接受更長時間且持續的正念練習,以達到情緒調節且改善注意力控制。 Chronic pain patients diagnosed with fibromyalgia (FM) frequently suffer from persistent pain and challenges with cognitive control. This "dyscognition" is likely due to attentional resources being taken up by their pain experience. Mindfulness-based interventions have demonstrated positive effects on managing pain and enhancing cognitive and emotional regulation. Yet, research examining the relationship between mindfulness and executive attention in healthy individuals and those with clinical diagnoses has yielded inconsistent findings. Some propose that mindfulness interventions might have a more pronounced impact on attentional control for those with attention deficits compared to healthy controls; however, this perspective needs further investigation. Additionally, studies have found that the effectiveness of mindfulness training on executive attention may be influenced by emotional regulation and pain. To explore this, this study implemented a 4-week mindfulness-based stress reduction (MBSR) training for both fibromyalgia patients (FM) and healthy controls (HC) to explore changes in attention, emotion, and pain perception. The study utilized the counting Stroop task, the affect labeling task, and pain questionnaires to assess differences before and after mindfulness training. The results revealed that after mindfulness-based intervention, the healthy control group showed a reduction in reaction time in the counting Stroop task and a decrease in reaction time in recognizing emotions compared to patients. The differences in reaction times in attention and emotion tasks between the two groups may be attributed to two underlying factors: (1) different pain and emotional interference, and (2) distinct mindfulness traits. Firstly, the ongoing pain and emotional symptoms in fibromyalgia patients continue to pose challenges during cognitive tasks. Secondly, lower mindfulness traits in fibromyalgia patients might diminish their capacity to be aware of the present moment and embrace the acceptance aspect of mindfulness. Therefore, fibromyalgia patients may benefit from longer and sustained mindfulness practice to achieve emotional regulation and improve attention control. |
