摘要: 台灣醫療經歷1995年全民健保上路及1998年的總額支付制,此重大變革就政策、財務方面,影響了後來醫療院所的經營發展。2010年起,衛生署(現衛生福利部)開啟PGY訓練計畫,為六年醫學系教育加上畢業後兩年一般醫學訓練制度,此計畫在教育訓練上亦帶給醫學中心、醫院、基層診所的制度規劃一定的影響力。獨立的基層診所逐漸發展為合夥經營,形成聯盟、連鎖營運的形式。
本文以開業近三十年之獨資型牙醫聯合診所為例。個案診所於開業期間,陸續面臨上述產業環境的轉變。文內以資源基礎理論模型探討個案是如何在大環境的變遷下,在次專科領域中維持自身的優勢並順利轉型。從回顧過程中,省視過去轉型的時機、方式,擬定未來的經營方向。
近期的牙醫診所經營策略研究,多以連鎖牙醫體系為主,少有獨立經營之聯合診所的篇幅。一方面是朝連鎖體系發展的診所增加,另一方面是連鎖體系更重視經營管理層面。然而,開業牙醫師在進入市場時,多數仍以獨立或合夥經營為主。本研究藉由個案回顧,在揭示多年經營發展的同時,亦期許為新創牙科在找尋定位上提供一些啟發。
In 1995 and 1998, Taiwan’s medical care experienced a major change in the introduction of national health insurance and the global budget system, which affected the development of medical institutions in terms of policy and finance. Since 2010, the Department of Health (now the Ministry of Health and Welfare) has launched the PGY training program, which is a six-year medical education plus a two-year general medical training system after graduation. This plan also brings a certain influence on the system planning of medical centers, hospitals, and grassroots clinics in terms of education and training. Independent grassroots clinics have gradually developed into partnerships, forming alliances and chain operations.
This article takes the sole proprietorship dental clinic that have been in business for nearly 30 years as the research object. During this period, they faced the above-mentioned changes in the industrial environment one after another. This article will discuss how the case maintains its own advantages and smoothly transforms in the sub-specialty field under the changing environment. From the review process, look at the timing and method of past transformation, and draw up the future business direction.
Most of the recent studies on the business strategy of dental clinics focus on the chain system, and there is little space for independently operated clinics. On the one hand, there are more clinics developing towards the chain system, and on the other hand, the chain system pays more attention to the operation and management level. However, when dental practitioners enter the market, most of them still operate independently or in partnership. This study, through a case review, not only reveals the business development over the years, but also hopes to provide some inspiration for the positioning of beginner.
