摘要: | 到院前心肺功能停止的病患若沒有及時搶救或接受治療,可能會造成死亡,就算在病患康復後,也可能伴隨一些副作用,輕則使病患記憶力衰退,重則可能變成植物人,因此,要縮短到院前心肺功能停止的患者出現後,到其接受急救的這段時間,一直都是很重要的議題。其中,急救的方式除了心肺復甦術之外,配合使用自動體外除顫器,也可以提升到院前心肺功能停止患者的存活率。 在過去的研究中,學者們提出許多有關於如何加快運送AED的方法,像是透過計程車或是飛行器來運送,還有一些研究是關於尋找CPR急救人員的方法,除了專業的救護人員之外,讓旁觀者進行急救或是透過緊急救護app讓一般民眾共同協助,都能夠縮短到院前心肺功能患者出現後,到其接受急救的這段時間間隔。 本研究是以Uber-Eats服務熱點區域餐廳中位置的資料作為Uber-Eats車手的據點,同時考量了Uber-Eats車手的反應率,來評估在車手協助到院前心肺功能停止患者進行CPR和取得AED的情況下,能縮短多少到院前心肺功能停止患者接受到急救的時間間隔。 資料上,我們使用了台北市2017年到2019年共11660筆OHCA資料,還有截至2021年衛生福利部網站上有登記的AED資料,共2321筆,另外在Uber-Eats的官方網站上,下載了台北市的熱點區域的資料,共25個熱點區域,並用Google Place ID API來尋找熱點區域內的餐廳和飲料店,總共有418間,研究的結果顯示,在Uber-Eats車手的反應率為10%時,取得AED的反應時間可以加快大約1分鐘,CPR的反應時間則加快了大約1.8分鐘,在Uber-Eats車手的反應率為100%時,AED的反應時間可以加快大約2.5分鐘,CPR的反應時間則加快了大約3分鐘。綜合以上結果,我們認為Uber-Eats若能加入現有的緊急救護系統,對於不論是進行CPR的搶救還是協助取得AED皆有所幫助。 If OHCA patient is not rescued in time, it may cause death. Even after the patient recovers, there may be some side effects, such as memory loss or in a vegetative state. Therefore, shortening the time to receive first aid has always been an important issue. First aid methods include cardiopulmonary resuscitation (CPR) and the use of automatic external defibrillators (AED), both improve the survival rate of OHCA patients. In previous studies, scholars have proposed methods on delivering AED, such as by taxi or aircraft, and some studies have been about the way finding CPR responders. In addition to relying on emergency medical personnel, bystanders or the public to assist in first aid can shorten the response time. In this study, we used the restaurant location data in Uber-Eats service hotspot areas as the departure points of Uber-Eats drivers, and also considered the response rate of Uber-Eats drivers to evaluate how much response time can be shortened if the Uber-Eats driver assists CPR and AED access in OHCA incidents in current emergency medical service. We used 11,660 OHCA data from Taipei City from 2017 to 2019, as well as 2,321 AED data registered on the website of the Ministry of Health and Welfare as of 2021. In addition, 25 Uber-Eats hotspot areas data in Taipei City is downloaded on the Uber-Eats official website. And Google Place ID API is used to find restaurants in the hotspot areas, a total of 418 data. The results show that when Uber-Eats is at 10% response rate, the AED response time is about 1 minute faster, CPR response time is about 1.8 minutes faster, and at 100% response rate, the AED response time is about 2.5 minutes faster, and CPR response time is about 3 minutes faster. According to the results, we believe that if Uber-Eats responders can be added to the current emergency medical service, it will be helpful for OHCA incidents on shortening the CPR response time and AED response time. |
