| dc.description.abstract | 近年傷口研究主要趨勢聚焦於監控和介入因免疫系統不全,而難以癒合的傷口。對於這類傷口,醫院通常會以抗生素協助患者,但近年因抗藥性提升,導致其對慢性外傷的治療效果漸不理想。因此,以麗蠅為材料的蛆療法,成為治療慢性傷口選擇之一。蛆療法將麗蠅幼蟲以腐肉為生的特性應用於治療外傷,原先被視為另類醫療手段,現今則逐漸被重視。本研究之目的為藉由組織病理學角度評估台灣常見種類,大頭金蠅(Chrysomya megacephala)幼蟲在不同齡期作為蛆療法材料的可能性。以燙傷造成試驗用鼠體表外傷3天後,分為4組:實施蛆療法24小時、48小時、72小時的實驗組,以及不處理傷口的對照組。再經過治療後,表皮組織將被取下並做成組織切片。結果顯示,有經過蛆療法的傷口,前期恢復和對照組相同,12天後卻恢復得比對照組快速、完整,代表蛆療法非即時促進傷口癒合,大頭金蠅幼蟲確實有作為蛆療法材料的潛力。將蛆放置於傷口內24小時和72小時,傷口恢復前期速度較放置於傷口內48小時快速;將蛆放置於傷口內24小時和48小時,傷口恢復後期速度較放置於傷口內72小時快速。此外,大頭金蠅幼蟲可以在傷口內成長,但成長速度不一致,且待在傷口時間的時間越長,存活率越低。大頭金蠅確實對於燙傷癒合有正面影響,若要在台灣的醫療系統推行本土物種蛆療法,這是一個可行性很高的物種。 | zh_TW |
