| dc.description.abstract | "研究背景及目的： 臺灣長照機構發展已40餘年，但目前臺灣的機構大多仍採取傳統化的經營，較少能提供真正個人化、以人為中心的照顧。美國自1990年代開始發展機構照護文化變革(Culture change)，其用意是要讓機構照顧去機構化，賦權給服務使用者可以參與決定自己生活與照顧方法，回歸以人為中心的照顧，這項文化變革獲得長照機構服務使用者甚至員工相當高的正向回饋及評價，同時也呈現更佳的照顧成果。因此本研究想以此文化變革所發展出來的「文化變革之特點自評表」為基礎發展，使用「顧客導向的文化變革特點」問卷來了解這些變革項目對台灣機構照顧使用者的期望及發展的重要性，期望藉此找出適合臺灣文化的去機構的照護模式，也進一步分析變革的重點項目。 研究方法： 本研究將美國「文化變革之特點自評表」翻譯調整為適合臺灣文化的「顧客導向的文化變革特點」問卷。針對南北各一家長照機構服務使用者進行立意取樣，以訪談形式進行問卷收集。並以描述性統計分析了解長照機構服務使用者對於各構面及題目的重要程度，以雙變量相關係數分析及複迴歸分析了解服務使用者的特性與各構面重要性的相關性，再以質性分析服務使用者對於長照機構文化變革的想法以及建議。 研究結果： 就描述性統計分析結果來看，長照機構使用者最重視「機構領導面」(4.09±0.29)，最不重視「照護面」與「環境面」，但其中使用者對時間自主(3.81±1.50)、自決權期待(3.88±1.33)還是高的。在相關性分析與複迴歸分析結果顯示服務使用者「年齡」越大，會覺得「家庭及社區面」越重要(β= 0.138 , p = 0.039)；「教育程度」越高，對於「環境面」的重視程度就會越高(β= 1.838 , p = 0.031)。 討論： 服務使用者對照護面的低期待與國外文獻不符，本研究發現是因為使用者不想增加工作人員的工作量或影響其他使用者的生活作息而屈就目前的照護模式。因此捨棄與日常生活最相關的「照護面」與「環境面」的重要性與期望。但其實他們並不是不在乎，可能在傳統機構式照顧的文化讓使用者決定進入機構時，心裡已經做好準備放棄一部分的自我需求與照顧期待。但是服務使用者內心還是在乎自身的權利及個別化照護，即便入住機構，還是希望能夠保有隱私權、自主權。因此本研究認為除了改善機構領導面之外，也可以先從「照護面」及「環境面」作為首要改革構面，滿足服務使用者對於照護及環境面的重視感，之後隨著服務使用者也開始重視其他構面時，再推動其改革。" | zh_TW |
