新興地區醫院所面臨的困境和發展策略-以F醫院為例

Abstract

新北市是台灣人口最多的城市，但是醫療資源相對地缺乏，其中新莊、五股、泰山地區人口將近60萬人，卻沒有醫學中心等級的醫院，因此重症患者往往需跨區就醫，十分的不便利，再加上此區人口結構逐漸老化，亟需一具規模的醫療機構來守護當地居民的健康。天主教輔仁大學醫學院已在地深耕30年，作育無數醫護人才，卻始終沒有自己的教學醫院供見實習教學。在種種的因素下，個案F醫院於是於2017年9月成立。此研究的目的即是為了解F醫院在成立3周年之際，所面臨的挑戰，以及可能的經營策略及發展方向。研究方法係利用質性研究方法中的個案研究法，針對個案F醫院進行研究，並藉由SWOT分析來整理個案醫院之內部優、劣勢，與外在機會與威脅，並嘗試提出對策，使個案醫院能永續經營。而由SWOT分析的結果看來，F醫院最大的內部優勢就是有明確的核心價值，以及醫學中心級的醫療團隊及設備；內部劣勢是位處於較偏遠的區域，且醫院本身硬體空間限制多，業務發展受限；但是鄰近區域無大型醫療院所，並且塭仔圳重劃區即將啟動，是外在機會的利多；而最大的威脅則是健保支付制度日趨嚴謹，影響醫院的經營。根據SWOT分析的結果，F醫院的發展策略應優先結合自身的優勢及外在的機會，來樹立F醫院的品牌；再順勢配合塭仔圳重劃區以建立F醫院生活圈，並擴大院區空間，追求永續經營；而在財務上，除了持續發展本業以增加醫務收入和醫務利益率，降低人事成本也是很重要的課題，還必需拓展非健保醫務收入及非醫務收入，以減少來自於健保的衝擊。

New Taipei City is the most populous city in Taiwan, but its healthcare resources are relatively scarce. Among the 29 districts, Xinzhuang, Wugu and Taishan have a total population of nearly 600,000, but there are no large medical institutions. Critically ill patients who live in these areas are suggested to seek more advanced medical services across districts. In addition, demographic statistics show population ageing regionally. Therefore, in September 2017, Catholic Fu Jen University has established its teaching hospital, F Hospital, in Taishan district. The purpose of the present study is to recognize the challenges faced by F Hospital, as well as to guide the direction for future development. We use the qualitative case study approach to assess F hospital. By incorporating the SWOT analysis, we examine the strengths, weaknesses, opportunities and threats of F Hospital and propose management strategies. The results of SWOT analysis are summarized as follows: (1) Strengths: definitive corporate core value and comprehensive medical team; (2) Weaknesses: backward transportation and insufficient space; (3) Opportunities: no potentially competitor nearby and Wen Zai Zhen urban renewal program; (4) Threats: current payment system of National Health Insurance (NHI). Based on the aforementioned information, several strategies are proposed. First, F Hospital should establish its own brand by combining strengths and opportunities. Second, it should build “the living circle of F Hospital” and expand its campus by joining Wen Zai Zhen urban renewal program. Financially, it is crucial to reduce personnel costs and increase the revenues outside NHI reimbursement and healthcare services.
