dc.description.abstract | 金黃色葡萄球菌是臨床上常見的格蘭氏陽性菌,屬於人體中的常在菌叢之一,同時也是造成伺機性感染的重要致病菌。因此金黃色葡萄球菌的感染對醫院及社區環境皆造成嚴重的問題。
金黃色葡萄球菌ST45在1990年代首次發現於德國,幾年後擴散至整個歐洲,亞洲地區則較少出現ST45相關的案例。然而在最近幾年,ST45卻出現在中國、香港、新加坡等許多亞洲國家。此外,在台灣許多長照中及護理之家更是嚴重受到ST45感染的威脅。為了瞭解ST45快速在亞洲及台灣地區快速出現的原因,我們針對ST45作分析。 從臺大醫院2000、2005、2010至2013及部分2014年的血液感染檢體中共收集了66株臨床菌株,也都初步鑑定為ST45(15株MSSA、51株MRSA)。基因型分析中我們做了agr、spa及SCCmec等分子型態分析,發現ST45中agr-spa-SCCmec Ⅳ-t1081-ⅤT的型態占了所收集的66株菌中最多的比例。另外也藉由PFGE鑑定親緣關係,發現可以初步將66株菌區分為t1081及non-t1081兩群,而在搭配年代分析發現spa type t1081所佔ST45的比例越來越高,因此想探討t1081是否有哪些特性能夠提升自己的生存優勢。 我們分析了葡萄糖或血漿誘發生物膜生成的能力、細菌抵抗環境中的壓力(高溫、低pH及血清中)、對於H2O2與whole blood的耐受性,不過這些在兩者中皆無顯著差異。因此我們進一步分析Staphylococcal protein A的表現,發現有所差異,同時在t1081的細胞壁蛋白中,α-hemolysin表現也較non-t1081高,這些因素可能是導致t1081較non-t1081在環境中能廣泛存在的原因之一。 | zh_TW |
