Abstract: | 對反芻動物來說,瘤胃蠕動對食物的消化吸收有著密不可分的關係,但有各式各樣的原因會影響瘤胃蠕動性,如瘤胃過酸、瘤胃鼓脹、酮病、腹膜炎、內毒血症、酸血症、低血鈣、低血鉀、粗纖維採食不足、精料過度攝取、運動缺乏等原因,都會造成瘤胃蠕動變慢,若發生嚴重的蠕動遲緩,甚至完全停滯將導致瘤胃過酸化,嚴重時可造成動物的死亡。
在治療手段上除找出病發原因外,也會以恢復蠕動機能為首要目標,如能在早期積極刺激瘤胃的蠕動,就能減緩瘤胃狀況的惡化,甚至可以自行恢復到正常狀態。 基於以上原因,如何以針灸方式刺激以增加瘤胃的蠕動功能,達到治療的目的,為本研究探討的目標。在中華傳統獸醫學可以針灸來調節脾、胃失能(例如:嘔吐、下痢),因此實驗以電針灸刺激幾個中獸醫學常用在治療脾胃失能病患的穴位,探討是否對於反芻動物的瘤胃也能夠起到相似的調節作用,本次實驗總共使用了24頭鎮靜阿爾拜因山羊,對足三里(ST36)、脾俞(BL20)、胃俞(BL21)與腰百會(Bai-Hui)四穴,分別進行電針灸刺激20分鐘,並同時使用經皮胃電圖儀與聽診器監控瘤胃20分鐘內收縮次數,實驗中發現電針灸刺激足三里、脾俞與胃俞也都能有效增加被鎮靜山羊20分鐘內的瘤胃收縮次數(ST36從7.2 ± 1.2次增加為11.5 ± 1.0次、BL20從6.2 ± 0.8次增加為12.7 ± 1.5次、BL21從7.8 ± 0.8次增加為12.3 ± 1.2次,n = 6,p < 0.01),另足三里穴明顯增加了被鎮靜山羊瘤胃收縮期的時間(ST36從12.0 ± 1.5秒顯著增加為22.5 ± 0.3秒,n = 6,p < 0.01 )。 根據本次實驗結果得知,電針灸刺激脾俞、胃俞與足三里能有效改變瘤胃蠕動性能,能提供一種在臨床上可應用的治療方式,達到增加瘤胃蠕動,避免瘤胃遲滯的發生。 Rumen is the largest digestive organ in the ruminants. For ruminants, rumen motility has a close relationship with digestion and absorption, there are a variety of reasons could affect rumen motility, such as rumen acidosis, ruminal bulging, ketosis, peritonitis, endotoxemia, acidosis, hypocalcemia, hypokalemia, lack of crude fiber intake, lack of exercise and so on, will cause rumen hypomotility, rumen stasis, or even complete stagnation. It often worsen the situation of patient, resulting in rumen acidosis, when become serious will cause death. Upon treatment, primary goal is to correct underlining disease, restore rumen function and motility. If we can promote rumen motility in the early stage, and then we can slow down the deterioration of animal’s conditions. Some times, patient could be cured without surgery and other more invasive treatment. In traditional chinese veterinary medicine, acupuncture is used as an alternative therapy for spleen and stomach dysfunction, the related clinical sign include vomit and diarrhea. We try to use of electroacupuncture (EA) at 4 acupuncture points stimulates rumen to motility and shorten gastrointestinal transit time via a parasympathetic pathway in sedated goat (n = 6). Dry needles were inserte at bilateral ST-36 (lower limb) or BL-20 (back) or BL-21 (back) and electrically stimulated at 3Hz ( 15 Vp-p ) for 20 min. The result of EA at ST-36, the number of constraction within 20 min ( from 7.2 ± 1.2 times to 11.5 ± 1.0 times, n=6, p < 0.01), BL-20 ( from 6.2 ± 0.8 times to 12.7 ± 1.5 times, n=6, p < 0.01), and BL-21 ( from 7.8 ± 0.8 times to 12.3 ± 1.2 times, n=6, p < 0.01) significantly increased the number of constraction within 20 min compared with EA at Bai-hui ( from 8.0 ± 1.3 times to 7.8 ± 1.5 times, n=6, p < 0.01). The result of EA at ST-36, but not BL-20, BL-21, and Bai-hui, significantly increased the contraction time (Tc) ( from 12.0 ± 1.5 sec to 22.5 ± 0.3 sec, n=6, p < 0.01 ) at the rumen. compared with EA at BL-20 ( from 12.6 ± 0.9 sec to 9.9 ± 0.5 sec), BL-21 ( from 11.3 ± 0.5 sec to 9.5 ± 0.5 sec ), and Bai-Hui ( from 11.1 ± 0.5 sec to 9.4 ± 0.6 sec) . In the present study, EA was proved to have effects on ruminal motility. The result may provide another treatment option when ruminal motility was compromised. |
