dc.description.abstract | 研究背景
靜脈輸注是複雜且危險的給藥流程,同時給藥可能產生物化不相容反應,造成病人的傷害甚或死亡。過去的觀察性研究發現潛在靜脈注射藥品不相容事件發生率約為7.2%-18.7%,佔所有用藥疏失的4.8%-25%。多數的用藥疏失可被預防,如能在給藥流程早期發現並阻止錯誤的發生,即可減少病人的傷害。目前靜脈注射藥品不相容性研究多屬觀察性研究,其分析對象較少且時間較短;現今電腦醫令與護理給藥形成閉迴路式醫療管理系統(closed-loop medical management sys-tem),資料庫分析或許可提供長時間且病人數較多的資料。 目的 藉由臺大醫院內科、外科與兒科加護病房(intensive care unit, ICU)病人之電子醫令(computerized physician order system, CPOE)與護理給藥紀錄(electronic medication administration record, e-MAR)資料庫回溯性分析病人用藥組合中發生潛在靜脈注射藥品不相容事件之比率,找出ICU常見的潛在不相容藥品配對及易與其他藥品起不相容反應之藥品,並以e-MAR做為對照,了解藉CPOE研究靜脈注射藥品不相容性的可行性。 材料與方法 本研究使用臺大醫院CPOE與e-MAR資料庫並以MicromedexR文獻資料做為分析基礎,藉Microsoft Access 2010、Microsoft Excel 2010和SAS 9.2進行統計分析。 本研究室在2009年的衛生署計畫中已建置一靜脈輸注藥品不相容性資料庫,為進行本資料庫分析,除原收載之「不相容」藥品配對資料外,另增加「注意:易變化」分類中的不相容資料。該資料庫中收載藥品學名、濃度、稀釋液且y-site接觸不相容之藥品配對。 由CPOE資料庫中擷取2009年7月1日至12月31日間所開立之醫令檔,收納共13個ICUs、給藥途徑為靜脈輸注(cIF、IF、IV)之注射劑。排除靜脈營養製劑、稀釋液、特定給藥方式(heparin lock與CVVH)、未執行醫令及不完整資料後,利用PROC SQL語法將資料庫串聯並找出用藥時間重疊的藥品配對。以醫令起訖時間推估用藥期間,再與靜脈注射藥品不相容性資料庫比對後可篩選出所有不相容藥品配對、計算潛在靜脈注射藥品不相容事件發生率並找出ICU中常見的潛在不相容藥品配對與常發生不相容反應的藥品。 由CPOE資料庫中擷取2011年8月1日至9月30日間所開立之醫令檔,再藉醫令序號串聯e-MAR資料庫後能獲得護理給藥紀錄檔。護理給藥紀錄檔採用與醫令檔相同的收納排除條件,唯以病人實際的給藥時間取代醫令起訖時間推算用藥期間是否重疊。比較2011年醫令檔與護理給藥紀錄檔的結果以驗證用電子醫令研究靜脈注射藥品不相容性之可行性。 研究結果 ICU中,在不包含靜脈營養製劑與稀釋液的情況下,每人次住院使用6-9種靜脈注射藥品,發生3-6種潛在不相容藥品配對,潛在靜脈注射藥品不相容性事件發生率14%-20%。最常產生潛在不相容性的藥品配對包括albumin-midazolam HCl、ampicillin sodium-gentamicin sulfate、cefazolin sodium-dopamine HCl、cefazolin sodium-famotidine、cefazolin sodium-magnesium sulfate、dopamine HCl-sodium bicarbonate、famotidine-furosemide以及、furosemide-heparin sodium、furosemide-midazolam HCl等。Furosemide是ICU中使用人次最多亦最常與其他藥品發生不相容反應的藥品,其他如ampicillin sodium、ampicillin/sulbactam、cefazolin sodium等也都是頻繁發生潛在不相容反應的藥品。病人使用越多藥品,其發生潛在注射藥品不相容反應的可能性就越大。 使用醫令檔推估潛在不相容事件的發生次數並不一定準確,其正確預測值(positive predictive value, PPV)為64.2%;敏感度(sensitivity)52.9%。 討論 在考慮潛在藥品不相容性時使用醫令檔計算之次數可能會受到病人用藥數、住院時間長短等因子影響,以每人次住院中所發生之不相容藥品配對種類數來表示較不易受到影響。在易產生不相容反應之藥品方面,因藥品的使用頻率與發生潛在不相容反應的比率皆具有其臨床意義,故綜合考量後發現furosemide為使用人次最多亦最常出現不相容反應的藥品。本研究並未探討稀釋液與濃度對不相容反應的影響,但稀釋液與濃度的不同皆可能影響兩藥品間的相容性。目前藥品相容性資料仍不敷臨床需求,背景資料的缺乏可能會低估潛在注射藥品不相容事件的發生率。 結論 這是第一個以資料庫回溯性分析ICU中常見潛在靜脈注射藥品不相容性的研究。結果顯示每名住進ICU且使用兩種藥品以上的病人皆發生3-6種不相容反應;其中又以furosemide與其他藥品發生不相容反應的頻率最高。以e-MAR做為對照,由CPOE中可發現ICU常見之潛在不相容藥品配對其陽性預測值(positive predictive value)64.2%,敏感度52.9%。 | zh_TW |
