摘要: | 成功的牙髓病及補綴治療仰賴牙齒和補綴物之間有良好的鍵結。在眾多鍵結強度測量的方法中,推離鍵結測試法是最被廣泛應用於測量根管封填材料和釘鑄之間的鍵結強度。然而推離鍵結測試法是否適合應用在牙科學研究鮮少被討論。本論文的目的是要探討推離鍵結測試法在牙醫學研究上的可行性,並且提出一個新的測試方法來測量根管內材料的鍵結強度。
在第一部份中,我們應用三維有限元素法配合內聚力模型來模擬推離鍵結測試法的過程,並進一步研究介面的性質與實驗設計方法相對於推離力量的關係。結果發現推離鍵結測試法傾向是測量材料的固持力,而非單純的鍵結強度。當根管內材料的彈性係數較低時,如大多的根管封填材料,壓頭的直徑會影響推離力量的大小,而應該被標準化。本實驗建議壓頭的直徑約為0.65到0.9倍的填充材料的直徑。當利用推離鍵結測試法來比較不同根管封填材料的鍵結強度時,有可能超過推離鍵結測試法本身的力學限制,因而得到不準確的結果。因此,我們需要一個新的測試法來更精確的測量根管內材料的鍵結強度。 在第二部份中,我們提出一個新的鍵結強度測試方法:利用一個包含牙齒及釘鑄在中間的樹脂圓盤,於直徑方向加壓到斷裂。實驗中利用音射檢測系統與數位影像相關方法來確認這種鍵結強度測試法用於測量的牙本質和釘鑄之間的鍵結強度的可行性。我們發現牙本質和釘鑄之間的介面破壞會導致複合圓盤的破裂。而且最終測得的破壞力量可以被用來計算介面的鍵結強度。這種鍵結強度測試方法有以下幾點好處:樣本備製簡單、備製過程中失敗較少、斷裂模式的一致性、實驗間的變異也較小。 在最後一部分,我們使用有限元素分析來分析新測試方法的應力分布,以及模擬牙本質與釘鑄之間的介面破壞情形。結果顯示在新的鍵結強度測試方法中,牙本質與釘鑄之間的去鍵結會比試片的斷裂更早發生,驗證了實驗觀察的結果。最後我們提出對應於傳統巴西圓盤測試公式的修正因子,來形成一個簡單的鍵結強度計算方式,方便之後新測試方法的應用。 經過這一系列的研究,我們對於推離鍵結測試法應用在測量根管內充填物與牙本質的鍵結強度上,提出一些警告與建議。另外我們提出了一個新的鍵結強度測試方法,藉由實驗與有限元素法,證明了這個新鍵結強度測試法的可行性與機轉。 The success of endodontic and prosthodontic treatments depends on the adequate adhesion between restoration and teeth. Among existing methods, push-out bond test had been widely used to measure the bond strength of intracanal filling materials and dental post. However, the suitability of applying push-out bond test to dental research was rarely discussed. The purposes of this work were to examine the suitability of push-out bond test, and provide a novel technique to measure the bond strength of intracanal materials. In the first part, we used a 3D finite element with cohesive zone model to simulate the push-out bond procedure. The influences of interfacial properties and experimental setups on the force needed to push the filler out were examined. We found that push-out bond test is more like to measure the retentive ability of filler, rather than the interfacial bond strength only. When using in intracanal filling materials with lower elastic modulus, the punch diameter should be standardized, which the suggesting range is between 0.65 to 0.9 times of filler diameter. It is beyond the limitation of test to compare the bond strength of root canal filling materials by push-out bond test. Therefore, an alternative test method to measure the bond strength is necessary. In the second part, we provided a novel dentin bond strength measurement technique, which using the resin composite disks, containing a slice of dentin with intracanal post in the center, under diametral compression until fracture. The feasibility of this technique to measure the post-dentin interfacial bond strength was evaluated by acoustic emission (AE) and digital image correlation (DIC). We found that the failure of the composite disk in diametral compression was shown to be caused by debonding at the post-dentin interface. And, the final failure load could be used directly to calculate the bond strength. The advantages of this method included simpler specimen preparation, no premature specimen failure, more consistent failure mode, and smaller variations. In the final part of this work, we used finite element method to analyze the stress distribution and to carry out the sensitivity analysis. Progressive post-dentin interfacial debonding was also simulated to better understand the failure process observed in experiments. The mechanism of this novel method was discussed and confirmed that interfacial debonding will take place prior to fracture in the dentin, which was correspond to experimental observation. The correction factors, which can be used in conjunction with the analytical solution for traditional Brazilian disk test, were provided to facilitate the adoption of the composite disk in diametral compression for bond strength measurement. After a serial studies on this work, we provided some suggestions and warnings in applying push-out bond test to measure the bond strength of intracanal filling materials. Also, a novel bond strength measurement technique was introduced and validated by a series of studies. |
