dc.description.abstract | 整形外科的專業為外形功能重建、顯微外科、傷口照護,以及美容手術。但隨著美容醫學 (下稱醫美) 產業流行,愈來愈多非本科的醫師投入醫美市場。為了區別醫美醫師專業與否,坊間出現許多「醫美課程證書」,證明醫師有參與過美容醫學課程。2019年衛福部修訂「特定醫療技術檢查檢驗醫療儀器施行或使用管理辦法 (下稱特管法)」,承認醫美課程證書可代表醫師專業,此舉引起整形外科譁然。如果一張幾小時的上課證明足夠代表醫療專業,那麼,長達六年的整形外科臨床訓練究竟是在學什麼?本研究將從整形外科住院醫師訓練的角度,探討「累積經驗」帶給醫師們什麼樣的知識。
完成整形外科的住院醫師訓練最少需要六年,台灣一年1100名醫學畢業生中僅有25個人可以成為整形外科住院醫師,可見其人力資源稀缺。但根據衛福部調查,由於住院醫師勞動環境不佳,高工時的問題降低醫學院畢業生升學意願,在調查中,以整形外科工時最長,平均達到102小時,甚至有住院醫師一週工作150小時的情形。如此艱困的學習環境,究竟希望住院醫師習得什麼?住院醫師們又是如何看待自己的身分? 本研究採用情感理論架構進行分析,以整形外科住院醫師學習時的心境為中心,了解他們如何將身體經驗轉變為知識。研究結果發現,住院醫師訓練首要強調團隊合作能力,因此需要醫師具備友善、可溝通、可信任的人格特質,確保手術氣氛和諧,以維護團隊工作品質;另外,住院醫師也需累積大量手術經驗,來培養短時間掌握人力、物力與未來狀況之決斷力。醫病溝通部分,住院醫師藉由累積大量病床工作經驗,提升解釋病情之能力,其中,住院醫師除風險溝通,亦需要學習掌握病人情感,增加溝通有效性。因此,傳統住院醫師訓練依舊有其不可取代性。研究亦發現,醫美產業界貶低住院醫師訓練之現象,使住院醫師對未來感到疲倦,失去成長動力,並加深厭世情緒。有趣的是,住院醫師身處在厭世的情緒中,為了接受壓力,發展出一套強調自嘲,不談夢想的新型人際相處模式。 | zh_TW |
dc.description.abstract | In orthopedics, plastic surgeons (PS) regard reconstructive surgery, microsurgery, and aesthetic medicine. However, with the coming of aesthetic medicine commercializing, not only PS but more doctors enter the market. In order to look into the aesthetic medicine profession, the government revise the regulations for aesthetic physician, but the outcome is unfair to PS. Therefore, as to wonder if there could possibly be any difficulties in PS knowledge of experience sharing and reusing to problem diagnosis, and how should the difficulties be resolved, which are both necessarily worth to discuss.
