dc.description.abstract | 自主神經病變(autonomic neuropathy)造成的胃輕癱(gastroparesis)是常見的於糖尿病(Diabetes mellitus, DM)患者的疾病,患者會有許多胃腸道症狀,如較長的胃排空時間、噁心、嘔吐、腹脹、痙攣、疼痛等。然而,在臨床診斷疾病時,缺乏客觀以及可量化的參數來評估這些疾病的嚴重性。因此,發展一套有效的和具有一致性的系統,來診斷自主神經病變與胃輕癱是一個至關重要的問題。本實驗的目的是探討(1)在胃部的不同區域(2)不同的性別,年齡,以及控制組和糖尿病患者之間,這些因素是否會對於胃部黏膜神經支配有所影響。我們的實驗採用免疫組織化學,並利用神經纖維結構蛋白(protein gene product, PGP 9.5)作為神經標記物,接著量化位在三個不同的胃部區域,即幽門竇 (pyloric antrum),胃體 (body),胃底 (fundus),內的胃黏膜的神經纖維密度。研究對象為年齡和性別配對的對照組和糖尿病患者,兩組各為 14人(男性8例,女性6例)。胃黏膜神經支配因不同組別和不同的胃部區域而有所不同。在正常人胃部的不同區域中,神經支配的密度上並沒有太大的差異。然而,在胃幽門竇及胃底部,女性的黏膜神經纖維密度(mucosal nerve fiber density, MNFDs)較高於男性。另外,在女性胃竇,黏膜神經纖維密度和年齡之間的具有相關性。研究發現在糖尿病患者的中,胃黏膜神經支配密度有較高於對照組的趨勢,並且在胃幽門竇中兩組有顯著差異。 利用這樣的技術來量化胃粘膜神經支配密度提供了一種未來在臨床上具有潛力的新方法用以診斷胃腸道症狀和疾病。 | zh_TW |
dc.description.abstract | Gastroparesis stems from autonomic neuropathy are common in patients who suffer from diabetic mellitus (DM) with gastrointestinal symptoms, such as longer gastric emptying time, nausea, vomiting, bloating, cramps, and pain, et al. However, in clinical diagnosis, there is a lack of objective and quantifiable parameters to assess these deficits. Therefore, the development of an effective and universal system to examine the autonomic neuropathy and gastroparesis is a crucial issue. We aimed to examine (1) patterns of gastric mucosa innervation density among different parts of the stomach and (2) the differences of gastric mucosa innervation density between different gender, ages, and controls and diabetic patients. We applied protein gene product 9.5 (PGP 9.5) as a nerve marker via the immunohistochemistry method, we quantified the nerve fiber densities in the gastric mucosa of three different region of the stomach, i.e. pyloric antrum, body, and fundus. There were 14 control subjects and diabetic patients (8 males and 6 females) under age-gender paired. The PGP 9.5(+) nerve fiber showed distinct pattern in gastric mucosa of different regions. The innervation pattern was similar among different part of the stomach. However, the mucosal nerve fiber densities (MNFDs) was higher in the female antrum and fundus than male. In addition, in female antrum, the high correlation between MNFDs and ages existed. Finally, we demonstrated that there was a trend of higher MNFDs in gastric mucosa of diabetic patients. This technique to quantify innervation density of gastric mucosa provides a new approach to assess disease of the gastrointestinal symptoms in diabetes. | en |
