在台灣,65歲以上老人約有50%主述有慢性睡眠障礙問題,而失眠之盛行率,隨著年齡增長。老人失眠更容易感到能量不足、情緒低落、煩躁、疲倦、焦慮、注意力不集中、記憶力差、反應速度變慢,與其他健康相關問題。 失眠常伴隨不正常的高度生理警醒,如交感神經系統活化。而降低生理活化之治療方法為增加副交感神經活化,抑制交感神經系統。研究指出,本體覺與深壓觸覺可改善軍事人員的戰鬥壓力症狀以及感覺過度反應之成人,即透過本體覺與深壓觸覺,提升副交感神經活性,抑制交感神經,減少焦慮、過度警醒與改善睡眠。研究亦發現,深壓觸覺治療可改善發展障礙孩童所伴隨之焦慮、害怕之情緒,以及讓正常孩童看牙科會感到放鬆。因此,放鬆會增加睡眠效率,例如減少睡前高度警醒、增加夜間睡眠時間、減少睡眠潛伏期,這些因素皆與恢復性睡眠息息相關。 迄今仍無研究利用深壓觸覺和本體覺介入於失眠老人,故本研究為第一個研究探討深壓觸覺和本體覺刺激能否有效改善老人之失眠問題 目的: 本研究探討深壓觸覺和本體覺刺激能否有效改善安養中心老人之失眠問題,幫助提升其睡眠品質,減緩睡前高度警醒,降低失眠頻率以及提升其生活品質。此外,若安養中心失眠老人伴隨憂鬱與焦慮問題,則進一步探討憂鬱與焦慮問題能否透過深壓觸覺和本體覺治療後得到改善。 方法: 研究設計:單盲,並以隨機分派方法將個案分成實驗組及控制組。 研究對象:來自於北部某間安養中心,50位六十五歲(含)以上之失眠老人。 治療介入:針對實驗組提供深壓觸覺與本體覺治療介入方案,一週介入五日,一日三次,一次八至十分鐘,持續二週。控制組則不提供任何治療。 成效量測工具:中文版雅典失眠量表、中文版匹茲堡睡眠品質量表、中文版睡前激發程度量表、臺灣簡明版世界衛生組織生活品質問卷 (WHOQOL-BREF (TW))、貝克焦慮量表中文版、中文短版老人憂鬱症量表、睡眠日誌(一週失眠次數)。 結果: 研究結果顯示,實驗組在雅典失眠量表 (p < 0.01)、匹茲堡睡眠品質量表中的主觀睡眠品質 (p < 0.01)與總分 (p < 0.05)、一週失眠頻率 (p < 0.01)、WHOQOL-BREF (TW)的心理領域 (p < 0.05)、環境領域 (p < 0.01)之改變量相較於控制組有顯著進步,且在雅典失眠量表 (ES=0.937)、匹茲堡睡眠品質量表中的主觀睡眠品質 (ES=0.944)、失眠頻率 (ES=0.918)與WHOQOL-BREF (TW)中的環境 (ES=0.822)之效果值達高度效應以上,而在匹茲堡睡眠品質量表總分 (ES=0.654)與WHOQOL-BREF (TW)中的心理 (ES=0.625),效果值達中到高度效應。但在睡前激發程度量表、WHOQOL-BREF (TW)中的其他面向,如整體生活品質、一般健康、生理健康與社會關係之前後測結果改變量,雖相較於控制組未達到顯著差異,但在實驗組組內前後測結果比較,發現睡前生理 (p < 0.05)與認知激發程度 (p < 0.05)、WHOQOL-BREF (TW)中的整體生活品質 (p < 0.05)達到顯著改善,而控制組組內各變項則皆未達到顯著差異。 結論: 本研究結果支持深壓觸覺與本體覺治療安養中心失眠老人之可行性,並提供一些證據表明此療法對於改善安養中心老人之失眠問題、提升其睡眠品質與改善生活品質以及減緩其情緒問題是一種有效的介入方式。 關鍵字:深壓觸覺、本體覺、失眠老人、睡眠障礙、睡眠困難 | zh_TW |
