dc.description.abstract | 瘧疾過去在聖多美普林西比民主共和國 (以下簡稱聖多美) 造成極大的威脅,在各種防治措施介入後,疾病發生率和盛行率都有顯著的下降。本研究藉由分析聖多美流行病學資料,驗證當地是否進入瘧疾清除前期。透過定量即時聚合酶鏈鎖反應和巢式聚合酶鏈鎖反應,比較分生檢驗與鏡檢結果差異。此外,將聚合酶鏈鎖反應陽性檢體進行瘧原蟲裂殖體表面蛋白第一型 (MSP1)和瘧原蟲裂殖體表面蛋白第二型 (MSP2) 基因定序,分析聖多美不同年代、地區、寄生蟲等級檢體中所帶惡性瘧原蟲流行株趨勢。研究蒐集西元2000年起聖多美13年發燒與瘧疾確診人數,以及2010年瘧疾陽性病人 (n=28)、2011年至2013年間低寄生蟲血症病人 (n=377)、不明發燒病人 (n=605) 與普林西比島全島篩檢 (n=6855) 之資料與血片,結果顯示僅有Lemba省和Principe自治區達到世界衛生組織訂立瘧疾清除前期標準。在鏡檢為瘧疾陰性的461位門診發燒病人中,有15人 (3.3%) 經PCR判定為感染瘧原蟲;普林西比島篩檢則分生檢驗與鏡檢結果一致。分析PCR陽性檢體之親緣關係發現,聖多美惡性瘧原蟲流行株大致可分為三大群,是否源自不同國家傳入其他型別則需進一步分析。藉由分子流行病學資料顯示,聖多美目前已逐漸進入瘧疾清除階段,本研究結果將提供聖多美瘧疾治療與疫苗發展的相關資訊,對未來瘧疾防治能做更適宜的政策規劃與準備。 | zh_TW |
