摘要: | 動脈粥樣硬化是鸚鵡極常見的心血管疾病。儘管鸚鵡動脈粥樣硬化的發生率很高,至今仍難以在鳥類生前診斷出來。又因為患鳥常缺乏明顯臨床症狀,往往在死後屍解才被發現,因此目前獸醫師們極欲了解如何早期診斷該疾病。在人類醫學,電腦斷層血管攝影已被廣泛應用於動脈粥樣硬化的診斷,被認為是診斷鳥類心血管疾病最具有發展性的診斷工具之一,也越來越多研究朝此方向發展。在人類醫學,調整顯影劑的條件能有效的改善影像品質,其中包含顯影劑注射速率以及注射總量之調整。本次研究中,我們使用10隻非洲灰鸚鵡(Psittacus erithacus)做為實驗對象,運用不同顯影劑注射速率0.3 mL/s、0.4 mL/s、0.5 mL/s,搭配不同的劑量740、370、222 mg of iodine/bird來比較顯影之成像品質。結果發現,在這三種不同顯影劑注射速率下,對於6條大血管所測得的直徑、衰減值、雜訊值、訊號雜訊比(SNR)、對比雜訊比(CNR)皆無顯著差異(p>0.05),但在速率越高的條件下,越能減少顯影劑分布異質性發生。而在這三種不同顯影劑注射劑量下,劑量越低,可以顯著降低其衰減值、雜訊值,並增加訊號雜訊比、對比雜訊比(p<0.05)。最後,我們建議臨床上使用370 mg of iodine/bird的劑量,因為其有良好的客觀與主觀影像品質;搭配0.4 mL/s或0.5 mL/s的注射速率,來降低顯影劑分布異質性發生。未來會需要更多臨床研究來佐證不同顯影劑注射條件下的診斷準確度。 Atherosclerosis is a common cardiovascular disease in parrots. However, antemortem diagnosis is considered challenging. In human medicine, computed tomography angiography has been widely used in the diagnosis of atherosclerosis. By adjusting the injection conditions, including the contrast injection rate and the total dosage, the image quality can be improved effectively. To test the effects of different contrast injection conditions on image quality of major arteries, 10 African grey parrots (Psittacus erithacus) were included as experimental animals. Three injection rates (0.3, 0.4, 0.5 mL/s) and three contrast dosages (740, 370, 222 mg of iodine/bird) were tested while the other variables of the studies were fixed. The results showed that there was no difference of the measured diameter, attenuation value, image noise value, signal-to-noise ratio, and contrast-to-noise ratio of the six major arteries (p > 0.05) among these three contrast injection rates; however, with the contrast injection rate of 0.4 mL/s or 0.5 mL/s, the incidence of heterogeneity decreased. In contrast, these parameters had significant differences among the three contrast injection dosages. The dosage of 370 mg of iodine/bird provides better subjective image quality than 222 mg of iodine/bird, and reaches optimal attenuation, lower image noise, higher SNR and CNR compared to 740 mg of iodine/bird. Above all, we recommend a combination of 370 mg of iodine/bird with 0.4 mL/s or 0.5 mL/s for clinical use. Further study is warranted to evaluate the diagnostic accuracy of images acquired with different conditions of contrast medium. |
