摘要: | 背景
台灣肺癌發生率逐年上升,但吸菸率和二手菸暴露率有所下降。亞洲地區,較少有世代研究指出居家暴露二手菸與肺癌的發生率之間的相關性。我們研究的目的旨在探討居家暴露二手菸與肺癌發生率之間的關係。 方法 我們使用2001與2005年的國民健康訪問調查進行了世代研究。共有41656名參與者被列入作為研究對象。肺癌的發病個案來自2001到2013年的台灣全民健康保險資料庫之重大傷病登記,使用Cox比例風險回歸模型進行分析,評估居家暴露二手菸和肺癌發病的關聯。 結果 在10.5年的追蹤中,共230名被確診為肺癌,分析中顯示居家暴露二手菸對於男性的肺癌發生率具有顯著相關。 結論 本研究顯示居家暴露二手菸和肺癌之間具有相關性,居家暴露二手菸與肺癌風險之間的關係尚不清楚,可能需要進一步研究。 Background Taiwan lung cancer incidence rate is increasing year by year, however the smoking rate and passive smoking exposure rate have decreased. Fewer cohort studies reported the correlation between second hand smoke and lung cancer incidence among never smoker in the Asia. The purpose of this study was to investigate the association between household tobacco smoke and lung cancer in never smoker. Methods We conducted a cohort study by using the 2001 and 2005 National Health Interview Survey. A total of 41656 participants were enrolled in the study cohort. The incident cases of lung cancer were identified from Taiwan National Health Insurance Research Database from 2001 to 2013.Cox proportional hazard regression model was performed to evaluate the risk of lung cancer in smoker and non-smoker with/without household tobacco smoke with adjustment of confounders. Results During 10.5 follow up years, 230 participants were diagnosed with lung cancer. Compare with group of non-smokers without household tobacco smoke exposure group, lung cancer incidence rate was significantly higher in smokers (HR 2.02, 95% CI 1.39-2.94, P 0.0002). In the male group, lung cancer incidence rate was significantly higher in smokers (HR 2.88, 95% CI 1.76-4.72, P <0.0001) and non-smokers with household tobacco smoke exposure group (HR 2.12, 95%CI 1.17-3.83, P 0.01). In the female group, the effect of smoking and household tobacco exposure were not observed. Conclusions In this cohort study, statistically significant relationship was found between exposures to household tobacco smoke for lung cancer in the male. The relation between household tobacco smoke and lung cancer risk is unclear and may require further study. |
