dc.description.abstract周邊神經系統的細胞本體位於背根神經節 (Dorsal Root Ganglion, DRG) 中,背根神經節內的初級感覺神經元在周邊神經損傷後,可以觀察到其內部的物質會有重新分配的情形;實驗室先前的研究發現,在正中神經損傷後,背根神經節中NPY (Neuropeptide Y) 免疫陽性反應神經元的數量會增加,calcitonin gene-related peptide (CGRP) 和substance P (SP) 的表現量則是會下降,而先前在坐骨神經損傷後發現,背根神經元中growth-associated protein 43 (GAP-43) 表現量會有顯著增加的情形。GAP-43是一種與生長相關的蛋白質,在一般的情況下,在背根神經節中表現量稀少,難以偵測;由於正中神經損傷後,背根神經節中物質的相對應變化的研究仍相當缺乏,所以本研究想以免疫組織化學染色的方式,觀察在正中神經截斷後,第六頸髓節背根神經節 (6th cervical dorsal root ganglion, C6 DRG) 中GAP-43、Y1R、Y2R、SP免疫陽性反應神經元的百分比的變化,再觀察糖尿病的誘發和局部麻醉劑的給予是否會影響GAP-43和SP的數量的改變,最後利用免疫螢光染色的方式觀察在正中神經損傷後2週及4週,GAP-43分別同時具有和NPY、Y2R、SP、CGRP、或nNOS的共同標誌的神經元之百分比變化情形。
結果顯示在正中神經截斷後,C6 DRG中GAP-43免疫陽性反應神經元比例會增加,在2週達到最大值,2週之後會開始下降,至5週和未損傷的組別在統計上沒有差異;Y1R的數量會在2週增加,其他時間點則與正常的組別沒有差異;Y2R的免疫陽性反應神經元比例在損傷的各個時間點與未損傷的組別在統計上並無顯著差異;SP免疫陽性反應神經元的百分比在神經損傷後下降。短期糖尿病並合併正中神經截斷處理的組別,GAP-43免疫陽性反應神經元比例在損傷後3週有最大值。在長期糖尿病合併正中神經截斷處理的組別,C6 DRG 中GAP-43免疫陽性反應神經元比例在4週上升;在短期糖尿病模式中,正中神經傷害對側可以觀察到SP免疫陽性反應神經元比例下降,而手術側DRG中SP的數量在長期或短期糖尿病合併正中神經傷害模式中皆沒有顯著的變化。正中神經截斷前15分鐘分別注射saline與1%、5%和10% lidocaine (LDC),結果顯示術前給予5% LDC和10% LDC與只進行正中神經截斷4週的組別相比,其DRG中GAP-43所佔的百分比有顯著的上升,而SP的百分比則在各個處理組別之間則沒有顯著差異。免疫螢光雙重標誌的結果顯示正中神經截斷後4週,同時表現GAP-43與NPY的神經元佔GAP-43免疫陽性反應神經元的百分比與2週相比顯著下降,正中神經截斷後4週同時表現GAP-43與Y2R的神經元佔GAP-43免疫陽性反應神經元的百分比與2週相比顯著上升,而正中神經截斷後4週同時表現GAP-43與SP、同時表現GAP-43與CGRP和同時表現GAP-43與nNOS佔GAP-43免疫陽性反應神經元的百分比與2週相比則沒有顯著差異。
正中神經截斷後背根神經節中GAP-43表現的百分比與坐骨神經傷害模式有所出入,坐骨神經傷害模式中GAP-43在背根神經節表現的時程較長,或許可以說明不同部位的神經損傷其再生情形不盡相同;在糖尿病的老鼠中,GAP-43的百分比會因應誘發糖尿病時期的長短而有所變化,顯示神經軸突的再生會受到高血糖環境的影響;在免疫螢光染色中發現GAP-43減少的同時,GAP-43與Y2R雙重標誌的神經元百分比增加,可能代表Y2R和GAP-43在細胞訊息傳遞上有所關聯。以上發現可能可以提供關於神經再生相關研究一些新的看法。
