dc.description.abstract由我們實驗室先前的研究結果得知,當正中神經損傷後其相應對側前肢可以觀察到有神經病變疼痛的行為表現,而且當受損的正中神經給予電刺激時,在同側的楔狀神經核中才可偵測到c-Fos免疫反應的神經元,並且有大量增加的情形。雖然許多研究認為c-Fos蛋白為周邊神經承受傷害性刺激所誘發產生,並且被廣泛認定為辨識神經興奮性或參與痛覺傳遞的指標。但是到目前為止,缺乏探討受損神經的興奮性是否與楔狀神經核中c-Fos的免疫反應神經元的數量有所關聯。因此我們想要以電生理胞外記錄技術分析,低電流強度電刺激正常或截斷損傷不同存活時程的正中神經,並記錄其個別所產生的複合型動作電位。定量分析各時期的神經複合型動作電位時,發現不論複合型動作電位的振幅、持續時間及其面積,這三項數值在神經截斷損傷後四週,分別為343.9 μV,7.6 ms及1.6 uV2s,相較其他各個時間都來得高。同時以低電流強度電刺激神經時,在楔狀神經核的c-Fos免疫反應的神經元數量在截斷損傷後四週同時也達到最高峰,意味著神經興奮性高低與楔狀神經核的c-Fos免疫反應的神經元數量多寡有正面相關性。
然而造成受損的正中神經其興奮性被誘發提高,是否與頸段背根神經節中神經元鈉離子與鉀離子通道表現量及其分佈位置等的變化有所關聯,則需研究加以釐清。因此探討周邊神經截斷後,神經損傷後不同時程的鈉離子與鉀離子通道的表現變化。利用免疫細胞化學染色法,探討其第六頸椎背根神經節當中,鈉離子通道Nav1.8、Nav1.9,以及鉀離子通道Kv4.2、Kv4.3免疫反應的數量變化。結果發現,在背根神經節中Nav1.8免疫反應神經元,其數量在神經截斷損傷後一週有下降減少的情形,但是在第二週又明顯回升,然而在第四及第五週時表現量又有再次下降的現象;由此得知正中神經截斷損傷時,在其第六頸椎背根神經節Nav1.8免疫反應神經元數量,以神經截斷損傷後第二週為高峰期,而近似於未損傷控制組。另外,有關Nav1.9免疫反應在損傷時間點的表現變化也有類似的情形。但是偵測Kv4.2免疫陽性反應產物,結果發現它們大多分佈在神經纖維上,而無法在神經元細胞本體偵測到。至於Kv4.3免疫反應神經元的數量變化,在實驗各組及正常組之間,經計量分析比較結果發現它們沒有統計上顯著地差異。綜合以上結果得知,電生理記錄截斷損傷正中神經興奮性的變化與鈉、鉀離子通道表現的時程變化雖然不完全吻合,但是與楔狀神經核的c-Fos免疫反應的神經元數量變化卻有相當的一致性,其可能的原因有待將來的研究進一步地探討。
