dc.description.abstract在台灣廣東血線蟲是引起人類嗜酸性腦膜腦炎的主因。目前治療上因顧忌給予殺蠕蟲藥會導致蟲體同時大量在腦部死亡,而死亡的蟲體會釋出蟲體抗原,可能造成腦部劇烈發炎,故多採用支持療法或症狀治療,例如:給予口服類固醇藥或抽取腦脊髓液降低腦壓以減輕病人的痛苦。因此我們想進一步探討在感染廣東血線蟲的鼷鼠給予口服殺蟲藥治療後,腦部發炎反應是否更為劇烈?透過計算鼷鼠腦內之蟲數回收數、觀察腦部組織病理變化及抽取部分腦部組織核醣核酸觀察特定細胞激素表現的變化,來評估治療是否會引發更劇烈的發炎反應。每隻鼷鼠口服感染30隻廣東血線蟲之第三期幼蟲,所有感染的鼷鼠在感染後的第十二天,分成四組分別為感染後不做處理組、單獨給予殺蠕蟲藥組 mebendazole (20 mg/kg of body weight)、單獨給予類固醇組dexamethasone (0.5 mg /kg of body weight)及同時給予以上二種藥物之共同治療組,持續給予口服藥至犧牲前一天。每組使用三隻鼷鼠。所有實驗組鼷鼠分別在感染後的第14、16、18、20、26、30天犧牲。組織病理切片顯示未處理的實驗組,呈現出典型的急性嗜酸性腦膜腦炎。殺蠕蟲藥組之蟲數回收數與未處理之實驗組相較下,在連續服藥六天及十四天後明顯下降,而腦部依然可見發炎情況。類固醇組之蟲數回收數則與未處理之實驗組沒有差異性,但腦部發炎反應有略微減緩的現象。共同治療組之蟲數回收數在連續治療六天後就有顯著性下降,且腦部發炎也有顯著性的改善。在腦部組織細胞激素核醣核酸的表現上,可觀察到IL-5的表現在未處理之實驗組有隨著時間增加的趨勢,而在共同治療組則在感染後第三十天時較第十四天時表現量降低。IL-12的表現量在感染後第三十天時的殺蠕蟲藥組和共同治療組表現量較為顯著。實驗的結果顯示出,在感染廣東血線蟲的鼷鼠腦部組織核醣核酸細胞激素的表現,在共同治療的過程中,免疫發炎反應有從Th2轉變為Th1的趨勢,且確實有改善腦部發炎的情形及其他組織病理性的傷害。zh_TW
