摘要: | 背景:低骨密度老人常有胸椎過度後凸,合併軀幹肌肉力量、步態、平衡表現的衰退,使跌倒及骨折的風險增加。然而胸椎過度後凸如何影響平衡穩定度仍未釐清,故須探討此生物力學關係,才得以設計有效之復健運動。過去研究已證實軀幹肌肉訓練能增進背肌肌力,但胸椎過度後凸者在訓練時常使用代償策略造成下背痛與壓迫性骨折。肌電訊號生物回饋已被證實能提升運動訓練效果,故本研究欲探討使用肌電訊號生物回饋儀用於軀幹肌肉訓練,是否能改善低骨密度者因胸椎過度後凸所帶來的負面影響。
目的:(一)探討胸椎曲度對於老人平衡穩定度之影響;(二)探討軀幹肌肉生物回饋訓練對停經後低骨密度者之姿勢與平衡控制的影響。 方法:研究一為觀察性研究,使用動作分析系統探討胸椎曲度與平衡表現之關聯。研究二招募停經後低骨密度者接受軀幹肌肉生物回饋訓練,並於訓練前後進行生物力學與臨床表現之評估。 結果:研究一,過度胸椎後凸組中,胸椎後凸角度越高者,其身體重心越往前;骨礦物鹽含量與背部伸肌肌力亦與平衡表現呈顯著正相關。研究二,停經後低骨密度者接受六周軀幹肌肉生物回饋訓練後,脊椎活動度、腹直肌與背部伸肌肌力增加,平衡表現與肌肉收縮模式亦有改善。 結論:生物力學分析結果顯示胸椎過度後凸之老人會有身體重心前移的現象,且背部伸肌肌力與骨礦物鹽含量會影響其平衡表現;而軀幹肌肉生物回饋訓練可改善軀幹肌力、姿勢與平衡控制,以期降低其跌倒與骨折之機率。 Background: People with low bone mineral density (BMD) often have thoracic kyphotic posture, and combined with decreased trunk muscle strength, poor gait and balance performance, thereby, increasing the risk of falls. The relationship between kyphotic posture and balance stability is unclear, and needs further investigation to prescribe effective rehabilitation exercise. Previous studies showed people with low BMD have increased trunk muscle strength after receiving trunk muscle stabilization exercise. However, compensatory strategy during exercise can lead to low back pain and compression fractures. Electromyography biofeedback training have been proven can enhance effect of training. Therefore, this study investigated whether trunk muscle training with electromyography biofeedback can improve thoracic hyperkyphosis in people with low BMD. Purpose: (1) To investigate the relationship between spinal curvature and balance stability in older adults. (2) To investigate whether trunk muscle biofeedback training can improve posture and balance control in postmenopausal people with low BMD. Methods: In study 1, a motion analysis system was used to investigate the relationship between spinal curvature and balance stability. In study 2, postmenopausal people with low BMD received trunk muscle biofeedback training. Clinical and biomechanical assessments were performed before and after training. Results: In study 1, a significantly positive correlation was found between the center of mass (COM)-center of pressure (COP) inclination angle and thoracic curvature in the high kyphotic group. Bone mineral content and back extensor strength also had significant correlation with balance performance. In study 2, the spinal mobility and trunk muscle strength of postmenopausal people with low BMD increased after receiving trunk muscle biofeedback training. Besides, the pattern of trunk muscle activity and balance performance were also improved. Conclusion: The results of this study revealed that the COM of older adults with thoracic kyphotic posture was closer to the limit of the base of support. Trunk muscle biofeedback training could improve the trunk muscle strength, posture and balance control of people with low BMD, thereby might decrease the risk of falls and fractures. |
