摘要: | 近年來台灣經濟生活水準提高,出生率持續下降,少子化與人口高齡老化已對人口結構帶來重大的改變,全民健康保險實施後,醫療環境在健保總額制度下,呈現同層級與不同層級醫院間,皆處在部分競爭部分合作之態勢。如何在健保總額制度下各醫療院所互相激烈競爭中,能持續增加醫療服務品質及廣度,同時也能提升服務業務量,建立一個創新性競合策略之價值網,以促進醫療同儕合作,提升醫療健康服務之能量,時為當今醫療界經營策略的重要議題。
個案醫院積極走入社區建構「社區健康營造網絡」,提供在地化健康服務,推動多元健康促進、整合式篩檢之預防保健模式,如健康體位管理追蹤,輔導地區餐飲商家推動健康飲食,社區健康地圖等,養成民眾自我健康管理行為。連結院內的淡水社區發展小組、院外的社區健康促進推動委員會之組織及設立59個社區健康服務站,作為院內外組織網絡平台,促進社區民眾身、心、社、靈的全人健康。 本研究透過麥可.波特的五力分析及第六力互補者等六種競爭力量分析,了解個案醫院在醫療產業中所處的競爭環境,及運用平衡計分卡管理工具點出產業中的競爭關鍵成功因素。從價值網分析中建立「社區醫療資源網絡」,各層級或不同專科的醫療院所不但是策略聯盟夥伴,如基層診所轉診,地區醫院之呼吸照護病房或腦中風病房轉介,精神專科醫院於內外科疾病照護的會診及病例討論交流等,事實上更是價值網內的互為互補者。以「社區醫療資源網絡」平台經驗提升友好賽局關係,建構個案醫院社區網絡醫療照護之價值網。 積極深入社區經營,不僅可配合目前國家醫療政策亦可拓展通路、擴大藍海市場,且屬非總額支付制度下,可增加營運收入。個案醫院於近幾年門急診日均服務量由民國96年1,197人次逐年增加,至民國104年已達1,930人次;轉診人次也由民國96年2,082人次至民國103年增加至4,409人次。此創新的商業經營模式如由醫學中心協力社區醫療機構以達成雙方之綜效,關係夥伴的選擇直接影響價值網絡的競爭優勢。如此,針對「社區健康營造網絡」及「社區醫療資源網絡」經驗建構個案醫院於社區醫療照護之價值網分析議題,得到完整與具體化之社區醫療照護網絡觀念,在國內醫療服務界面臨健保困境時,甚值得醫界參考。 In recent years, the economic booming in Taiwan has brought in problems such as declining birth rate and aging population, which have affected the population structure significantly. After the National Health Insurance(NHI) Program had been launched, the relationship between different hospitals, within or not within the same level, tends to be partially competitive and partially cooperative under the global budgeting of NHI. How to continuously broaden the range and promote the quality as well as the quantity of medical services under this global budgeting in order to build a new co-opetition value net, improve the cooperation between medical personnel, and enhance the energy for healthcare services, are important issues of today’s medical strategies. The hospital hereof did a nice job in building the “community health build-up network”, it provides local health cares and preventive health services for Multiple Health Promotion and Integrated Screening, such as conducting weight control and tracking, educating the local restaurants to provide healthy food, and marking exercise roads in the healthy commucity on special tourism maps. All these have helped people develop health self-management behaviors. As the platform of this Community Health Build-up Network in and out of the hospital, Community Development Team, Community Health Promoting Committee, and other 59 Health Stations, are doing their best for promoting community health of both body and mind. In this research, via Porter Five Force analysis and The Sixth Force of complementors analysis, we studied the industrial competitive environment of the hospital; with the help of The Balanced Scorecard, we pointed out the Key Success Factors for industrial competition; through the value net analysis, we build the “community medical resource network”. In this network, relationship of different hospitals, in different levels or of different expertise, is not only of strategic partner, but also mutually complementary for each other in the value net. Based on the “community medical resource network”, the hospital enhanced the relationship of friendly competitions, and built the value net of the community health network. Exploring the community health market not only supports the current national health policy, but also develops business opportunities and increases incomes within the global budgeting of NHI. The traffic of outpatient and emergency department of the hospital hereof has steadily increased from 1,197 in 2007 to 1,930 in 2015; the traffic of referral has increased from 2,082 in 2007 to 4,409 in 2014. In this innovative marketing system which combines the resource of medical centers and community health institutions to double the benefits, a good partner is the key to achieve competitive advantages within the value network. Facing the difficulties of NHI system, the analysis of building a community health value network by the hospital hereof based on the “community health network” and “community medical resource network”, and a comprehensive theory of such network, appears to be such valuable reference for the medical care industry in domestic now. |
