dc.description.abstract | 老年化社會是目前全球的嚴重問題。台灣目前已是老化最快速的國家,勢必面對老年化社會所代來的挑戰,例如社會與家庭結構的改變,與慢性病人口的快速增加。國際健康研究組織的報告指出超過65歲的族群中平均每人患有1.2個慢性病,例如高血壓、糖尿病,以及退化性疾病。面對這樣的情況我們需要有效的、有品質且經濟的個人化醫療照護。
研究顯示遠距居家健康照護(服務)可以處理老年化社會所帶來的照護問題。然而目前缺乏有組織性的研究方法來建置遠距居家健康照護系統,可能導致系統開發的缺失或失敗。台北市政府致力於促進健康社會政策,策劃台北健康城市計畫與持續推動 “活力健康市民”的願景。此研究中提出一個以結構(Framework)為基礎的研究方法以進行市民遠距健康服務系統建置的規劃。研究中所提出的方法可成為其它遠距健康照護系統建置計畫的範例。 方法架構中有三個階段,分別為早期偵測期、產生期與行動期。於早期偵測期我們文獻分析與專家訪問兩項質性研究,以確定局勢、可用資源、目標與限制。為了增進系統的可行性,老年人使用者研究於早期偵測期中執行。在產生期中使用的方法為情境分析與使用者問卷調查。初步測試與市民擴大測試於行動期中執行以利系統成效評估。 市民遠距健康服務系統的架構與服務鏈在研究中被提出,此系統直接與公共健康資訊與服務結合並成為台北市現行的政策之一。評估結果顯示使用者對於系統有正面評價。系統目前持續運行中,有收集到的健康與醫療資訊將來可作為醫療專家系統開發的基礎,以提升遠距健康照護系統的有效性。 | zh_TW |
dc.description.abstract | Aging society is a global problem nowadays. Being the fastest aging nation in the world, Taiwan is now facing challenges of society structure and house hold style changing, and expanses of health care demand. In a survey held by National Health research Institute (NHRI), elders with age over 65 have 1.2 chronicle diseases in average, such as hypertension, diabetes, and age related decayed. According to this situation, the need for efficiency, individualization and equity of quality-oriented health care with limited financial resources raised.
Research evidences showed that the tele-home healthcare system (services) has potential to deal with the challenges associated with the aging society. However, the lack of proper framework and guidelines leads to the weakness or failure for practical implementation. Taipei City Government (TCG) has devoted to promote health city policy and formed an integrated task force on Taipei Health City to work on the “Vital and Health Citizens” with vision of sustainability. In this study, a framework based research methodology was provided for planning and developing the Citizen Tele-health Care Service (CTCS) system, which can be a role model for other project of tele-home healthcare system development. There are three stages in the framework, which are the Early-detection stage, Generation stage and Action stage. In the Early-detection stage we used two qualitative methods, literature review and interviews, to indentify the current situation, resources, scope of the plan and limitation. In order to improve the usability of the system, the elder user study is necessary in the Early-detection stage. The methods used in Generation stage were scenario planning and questionnaire survey. In Action stage, pilot trial and citywide trial were conducted for evaluation. By using the framework based research method, the system architecture and services chain were provided, which were built with public health information and services and refer to the policy of Taipei city. The evaluation results showed that people have positive expectations and acceptance of CTCS system. The continually collecting of person health record in CTCS system can provide valuable data for the developing of effective expert system used in tele-healthcare field in the future. | en |
